Study Of The Relationship Between The Primary Caregiver's Cognition Of The ADL Training And The Elderly Stroke Patients' ADL Training

Objective: In order to explore the necessity and effectiveness of the improving of the stroke patients primary caregiver's cognition about the ADL training, so as to further understanding the correlation of the cognition and the elderly stroke patients ADL training, Researchers improve the primary caregivers, cognitive ability of the ADL training for 3 months, the nursing intervention was undertaken by the following of the caregiver, the nursing intervention can also improve the stroke patients ADL ability.Methods: Using convenient sampling, 51 stroke patients and their primary caregiver according to the selection criteria in the Fourth Hospital of Hebei Medical University and the second Hospital of Hebei Medical University. First, we evaluate the primary caregivers, cognition about the ADL training and the patients, ADL ability, based on the result of the evaluation, we give them the nursing intervention, after the 3 months intervention, we evaluate the caregivers cognition of the ADL training and the stroke patients'ADL ability again. The frequency of the intervention is for the first month, twice a week, the second month, once a week, the third month, Bi-weekly. The main content of the intervention is ADL training.Results: 51 stroke patients and their primary caregiver according to the selection criteria.1 The primary caregiver cognition of the ADL training before interventionThe primary caregiver's cognition of the ADL training was got by the communication with the caregivers, some people said they never heard of the ADL training, and 17(33.3%) of the caregivers think the ADL training will help the patient restore self-care ability. 31(60.8%) of the caregivers have not concerned about ADL training since the patient got stroke, 27(52.9%) caregivers think they would transform the family equipment only when the economic condition was well enough.The knowledge of the ADL training is also not very well, before the intervention, the correct answer rate more than 52% is the right height of the bed, the eating posture, the protection of privacy, the water depth of the tub bath, the height of the nozzle and switch when shower, the protection of the patient, the skill of wearing clothes or undress clothes and the way to the stairs, the correct answer rate of other problems is lower than 50%.2 The ADL ability of the patient before intervention The score of ADL ability before intervention is from 20 to 60, the mean score is 31.7±12.6, most of the ADL ability was on the state of severe dependence, and 17(33.3%) patients were on the state of total dependence, 23(45.1%) patients on severe dependence, 11 (21.6%) patients on moderate dependence.3 The primary caregivers'cognition of the ADL training after interventionAfter the intervention, the primary caregivers'cognition was improved greatly, 44(86.3%) caregivers think that the ADL training was important to the patients, and 35(68.6%) caregivers believe that patients can restore self management, most of the caregiver has mastered the knowledge of the ADL training, and they can guide patients in the right way.4 The ADL ability of the patient after intervention The ADL ability of the patients was improved greatly, the score of ADL ability before intervention is from 40 to 100, the mean score is 81.3±14.8, most of the ADL ability was on the state of mild dependence, 1(2.0%) patient was on the state of severe dependence, 3(5.9%) patients on moderate dependence. 41(80.4%) patients on mild dependence, 6(11.8%) patients could charge themselves.Conclusion: The elderly stroke patients ADL ability was in a higher degree of dependence, they need ADL ability training, however, investigations revealed that the majority of primary caregivers did not recognize the importance of the ADL ability of the importance of training, they have little knowledge on it, and they do not know the right way of the ADL ability training. So it is very important to improve the caregiver's cognition, then they will help the patient do the ADL ability training in the right way .and then improve the patients quality of life.
