ObjectiveObserve the effect of treating depression associated with astrointestinal disorders by acupuncture and moxibustion.MethodsThe target patients are from the Second Affiliated Hospital of Guangzhou University of Traditional Chinese Medicine.Random numbers were generated by PEMS3.1 software package.60 patients were successfully selected according to the admission standard and the patients were randomly divided into 2 groups with 30 cases in each group,which are the treatment group and the control group.The treatment group were treated by acupuncture and moxibustion(1) acupoints:Baihui,Yintang,Hegu(both sides),Taichong(both sides),Zhongwan, Tianshu(both sides),conventional acupuncture manipulations;(2) Moxibustion was applied on zhongwan and Tianshu(both sides),with 5 pieces of moxa on each acupoint.The control group were treated only by acupuncture.the acupoints selected and manipulating methods used were the same with the treatment group(1).Treatment courses:2 times per week,one treatment course contains 8 times of treatment,a total of three courses are required to finish the treatment(or the total number of treatment adds up to 20 times or more). t test was used to compare the differences before and after treatment in each group as well as the differences between the two groups.The efficacy index and score deduction rate were applied to evaluate effects at end of the treatment and the effects one month after the treatment.Chi-square test was used to compare the overall efficacy between the two groups and the rate of patients being cured or markedly effective.The HAMD,SDS and SCL-90 depression factor and SCL-90 somatic factor scoring before and after treatment,were used to give comprehensive assessment of the treating effect.Results 1.pre-treatment comparison on general information:In this study,60 cases were included,of which 21 cases are male and 39 cases are female.During the course of the study,one case in each group did not complete the observation due to personal reasons,therefore,the two cases were removed and were not included in statistics because their data was incomplete,Eventually 58 cases were included in the statistics.By X~2 test,comparing two groups of patients with gender,X~2=0.08,P=0.78>0.05;comparing two groups of patients with TCM syndromes,X~2=0.29,P=0.86>0.05.By t test,comparing two groups of patients with age,P=0.90>0.05; comparing two groups of patients with disease duration,P=0.82>0.05;comparing the two groups of patients with HAMD,SDS,scores of SCL-90 depression factor before treatment,P>0.05.Therefore,comparing two groups of patients before treatment by age, gender,disease duration and TCM Syndrome,as well as HAMD,SDS,and scores of SCL-90 depression factor and somatic factor,the difference was not statistically significant(P>0.05),indicating that the two groups are comparable.2.comparison of effects:At the end of treatment:among the treatment group,11 cases were cured, 13 cases were markedly effective,5 cases were improved,and 0 case was invalid, which means a total efficiency rate was 100%;among the control group,6 cases were cured,11 cases were markedly effective,12 cases were improved,0 case was invalid,which means the total efficiency rate was also 100%.The differences of the overall effect of the two groups by Ridit analysis were statistically significant(P<0.05);by X~2 test,comparing the rate of patients being cured or markedly effective(cure rate + significant efficiency rate) in two groups,the differences were statistically significant(P<0.05), showing that at the end of treatment,the treatment group had a better effect than the control group.follow-up one month after the treatment:among the treatment group,9 cases were cured,13 cases were markedly effective,5 cases were improved,and 2 cases were invalid,which means a total efficiency rate was 93.10%;among the control group,5 cases were cured,8 cases were markedly effective,9 cases were improved,7 cases were invalid,which means the total efficiency rate was 75.86%.The differences of the overall effect of the two groups by Ridit analysis were statistically significant(P<0.05),by X~2 test,comparing the rate of patients being cured or markedly effective(cure rate + significant efficiency rate) in two groups,the differences were statistically significant(P<0.05),showing that the treatment group had a better effect than the control group during the follow-up one month after the treatment.3.Assessment of the evaluation Scales:Comparison of HAMD Scale points:at the end of the treatment and the follow-up one month after the treatment,the HAMD scale points in both groups significantly decreased compared with that of pre-treatment(P<0.01), indicating that significant treatment effect was achieved in both groups for treating depression associated with gastrointestinal disorders;follow-up one month after the treatment showed that in both groups the HAMD scale points were slightly higher than the points acquired right after the treatment,the difference was statistically significant(P<0.01),which indicated the treatment group had a better effect than the control group during the following-up one month after the treatment.comparison of SDS scale score:at the end of the treatment and the follow-up one month after the treatment,SDS scale score in both groups significantly decreased compared with that of pre-treatment(P<0.01),indicating that significant treatment effect was achieved in both groups for treating depression associated with gastrointestinal disorders;the follow-up one month after the treatment,comparing SDS scale score of patients in two groups, the difference was statistically significant(P<0.01).comparison of SCL-90 scale somatic factor score:at the end of the treatment and the follow-up one month after the treatment,SCL-90 scale somatic factor score in both groups significantly decreased compared with that of pre-treatment(P<0.01),indicating that significant treatment effect was achieved in both groups for treating depression associated with gastro-intestinal disorders;the follow-up one month after the treatment,comparing SCL-90 scale somatic factor score of patients in two groups,the difference was statistically significant(P<0.01).comparison of SCL-90 Scale depression factor score:at the end of the treatment and the follow-up one month after the treatment,SCL-90 Scale depression factor score in both groups significantly decreased compared with that of pre-treatment(P<0.01),indicating that significant treatment effect was achieved in both groups for treating depression associated with gastrointestinal disorders;the follow-up one month after the treatment, comparing SCL-90 Scale depression factor score of patients in two groups,the difference was statistically significant(P<0.01).ConclusionsAcupuncture is a safe and effective therapy on treating depression associated with gastrointestinal disorders;acupuncture combined with moxibustion therapy is better than ordinary acupuncture group in both short-term and long-term effect.Symptom score and HAMD,SDS,SCL-90 score are relatively more sensitive.at the end of the treatment and the follow-up one month after the treatment,HAMD score,SDS score,SCL-90 Scale depression factor score and SCL-90 scale physical factor score in both groups all significantly decreased compared with that of pre-treatment(P<0.01), indicating that significant treatment effect was achieved in both groups for treating depression associated with gastrointestinal disorders;the follow-up one month after the treatment,comparing HAMD score,SDS score,SCL-90 Scale depression factor score and SCL-90 scale somatic factor score SCL-90 Scale depression factor of patients in two groups,the differences were statistically significant(P<0.01),indicating that the treatment group had a better effect than the control group.
