ObjectiveThrough a retrospective analysis clinical data of 118 Cases of adult atypical pulmonary tuberculosis,we analyze the diagnosis of adult atypical pulmonary tuberculosis,the misdiagnosed reasons and influencing factors.MethodsA retrospective analysis of 118 cases with confirmed adult atypical pulmonary tuberculosis patients.(male:78,female:40;aged froml4 to 85 years old).All patients were admitted to Sir Run Run Shaw Hospital,Medical College of Zhejiang University, during 2005-2008.ResultsMany patients had not typically clinical manifestations,28(23.7%) patients did not have any clinical symptoms;45(38.1%) patients had complications;68(57.6%) patients had isolated nodules or mass shadow in their chest imaging;2(1.7%) patients were diagnosed by the positive smearing of sputum acid fast stain;17(14.4%) patients' diagnosis relied on the histopathology by fibrotic bronchoscopes;3(2.5%) patients were diagnosed by combined the abnormal bronchial mucosa changes under fibrotic bronchoscopes with subsequently effective standard chemotherapy at least three months; 41(34.7%) patients were confirmed the histopathology by CT-guided lung biopsy;37 (31.4%) patients relied on the histopathology by surgery;18(15.3%) patients were diagnosed by combined pleural effusion examination with subsequently effective standard chemotherapy at least three months.ConclusionFor the adult tuberculosis patients whose manifestations are not typical,we should combine laboratory examination,imaging of chest,with the histopathology by fibrotic bronchoscopes or by CT-guided lung biopsy or by surgery.We should try our best to reduce misdiagnosis and improve the diagnostic abilities.
