Since the open policy, with China's rapid economic development, the total expenditure on health has also increased rapidly. In the meanwhile, the unreasonable structure of the total health expenditure increasingly becomes more prominent, it seriously affects the health development, also as a direct result of the"difficult and expensive medical treatment". Therefore, to solve the problems, the urgent task is to optimize the structure of total health expenditure.This paper expounds the concept, the structure and the influence of the total health expenditure, analyzes the source structure and the distribution structure of the total health expenditure, respectively, studies the coordination of health development and the economic development, total health expenditure per capita level, as well as the national, social and personal burden, the distribution of the total health expenditure between the urban and the rural areas, between the medical institutions and the public health institutions of China, in order to analyze deeply, the paper takes the data of Hubei province and Huanggang City as examples. In the course of the study, the paper focuses on the international data, current national health policy, health reform and development, as well as the"difficult and expensive medical treatment"which concerned by the whole society, so that the paper can be served by a new round of the national and local health reform. Also, the health outcomes are analysed to reflect China's health achievements. The conclusions includes: China's total health expenditure grows rapidly, but the accounts for the proportion of GDP is not high; it is unfair that the out of pocket spending is higher than the total of the government and the social spending; the distribution of the total health expenditure between the urban and the rural areas, between the medical and the public health institutions needs to be improved; the compensation of the medical and public health institutions is too low that they are hard to survive and serve; the costs of the inpatient and the hospital checks are too high that they excess the capacity of individual residents paid. The proposals are: strengthen macro-control, so that the health development can be in line with the national economic development; increase the government and the public health spending, reduce the propotion of the out pocket spending; rationally allocates the health resources, adjusts the use of the total health expenditure; increases health care funding Compensation, to maintain the health a sustainable development; raises the prices of medical services and increases the transparency of the prices and so on.
