Objective To study clinical characteristics of mycoplasma pneumonia combined with kidney impairment in children.Methods We collected signs and symptoms and clinical indexes of 584 hospitalized children with acute glomerulonephritis(divided into three groups:mycoplasma pneumoniae associated glomerulonephritis,post streptococcal glomerulonephritis and others infection glomerulonephritis) and 118 cases of nephrotic syndrome(divided into mycoplasma pneumoniae associated nephrotic syndrome and primary nephrotic syndrome),and comparatively analysed their clinical manifestations,laboratory examination results.Results Of 702 case,128 cases suffered with mycoplasma pneumonia combined with kidney impairment,accounting for 18.23%.Of 584 case children with acute glomerulonephritis,116 cases(19.86%) suffered mycoplasma pneumoniae associated glomerulonephritis.Of 118 cases with nephritic syndrome,12 cases(10.17%) suffered with mycoplasma pneumoniae associated nephritic syndrome.Clinical manifestations of mycoplasma pneumonia combined with kidney impairment was no-specific.Clinical manifestations of mycoplasma pneumoniae associated glomerulonephritis was similar to acute poststreptococcal glomerulonephritis.Compared among mycoplasma pneumoniae associated glomerulonephritis,acute poststreptococca glomerulonephritis and other pathogeninduced glomerulonephritis,there were no statistical significant differences in age of onset,the degree of hematuria and proteinuria,erythrocyte sedimentation rate,blood urea nitrogen,edema subsided time,convalescence of blood pressure(SBP) and proteinuria(P>0.05).But there were significant differences among three groups in convalescence of serum C3 and hematuria(P<0.05).Clinical manifestations of Mycoplasma pneumoniae associated nephrotic syndrome were similar to primary nephrotic syndrome.Compared of mycoplasma pneumoniae associated nephrotic syndrome and primary nephrotic syndrome in incidence of hematura,there was no statistically significant difference(P<0.05).But there was significant difference betweent two groups of nephrotic syndrome in the age,oliguria,duration of edema and proteinuria(P<0.05).Conclusion(1)Mycoplasma pneumoniae associated kidney impairment was one of the main causes of kidney damage in children of West of Guangxi,physician should be take more attention to.(2)Clinical manifestations of mycoplasma pneumonia combined with kidney impairment is no-specific, mycoplasma pneumoniae associated kidney damage mainly resulted in glomerulonephritis.Clinical manifestations of mycoplasma pneumoniae associated glomerulonephritis was nospecific in acute nephritis.However,the occurrence of manifestation of typical acute nephritis is lower than that in acute poststreptococcal glomerulonephritis,occurrence of hematuria and low complement were higher than those in acute post streptococcal glomerul- onephritis.The level of serum C3 in mycoplasma pneumoniae associated glomerulonephritis reduced,and it was higher than the level of serum C3 in acute poststreptococca glomerulonephritis,but lower than those in others infected nephritis.The convalescence of hematuria was shorter than those of post streptococcal glomerulonephritis and others.(3)Clinical manifestations of pneumoniae associated with nephrotic syndrome was nospecific in nephritic syndrome,but the age of mycoplasma pneumoniae associated with nephrotic syndrome was older than primary nephrotic syndrome.The incidence of oliguria was less,and the duration of edema,proteinuria were shorter than the latter.Mycoplasma pneumoniae associated with nephrotic syndrome recover more rapidly.It was sensitived to incretion and had good prognosis.
