Association Between Serum Resistin,Leptin And Carotid Atherosclerosis In Essential Hypertension Patients

Objective: Resistin is an adipocytokine belonging to the family of cysteine rich secretory proteins.Leptin is a 167-aa secreted protein encoded by the ob gene.Human leptin is mainly produced and secreted by white adipose tissue.It is a pleiotropic molecule with a wide range of biological actions 。First,to study the changes about serm resistin,leptin levels in hypertensive patients.Second,we sought to determine if a correlation between resistin,leptin levels and carotid atherosclerosis exists in hypertensive patients. We may clarify atherosclerosis pathogenesis in patients with hypertension, to contribute to the prevention and treatment about atherosclerosis.Methods: This study consisted of 272 hospital hypertensive patients and 90 cases of healthy controls. Serum resistin,leptin concentration were measured by enzyme linked immunosorbent assay(ELISA) in all subjects.Meanwhile,carotid intima media thickness(IMT) were measured by ultrasonographic in all subjects.All hypertensive patients were grouped into tertiles according to their resistin level;another grouping method, all hypertensive patients were grouped into tertiles according to their leptin level. Then, Multiple linear regression analysis for independent determinants of carotid IMT was run, resistin,leptin were the independent variables in the process.Results:1,Serum resistin,leptin were significantly higher in the hypertension group than the control group (resistinhypertension-group = 7.092±5.365ng/ml, resistincontro group=4.578±2.656ng/ml,Presistin=0.041; leptinhypertension group=24.653±16.297ng/ml, leptincontrol group=17.244±11.970ng/ml,Pleptin=0.013. Carotid IMT was significantly higher in the hypertension group than the control group(IMThypertension group=0.67±0.13mm,IMT(controlgroup=0.56±0.09mm,PIMT=0.003 ; maxIMThypertension group=1.27±0.24mm, maxIMTcontrolgroup=1.05±0.16mm,PmaximumIMT=0.002.Resistin levels were associated with the carotid IMT(carotid IMT:r=0.313,P=0.005 and maximum carotid IMT:r=0.277,P=0.013) in essential hypertension patients.Leptin levels were also associated with the carotid IMT(carotid IMT:r=0.394,P=0.000 and maximum carotid IMT:r=0.440,P=0.000)in essential hypertension patients.2,All hypertensive patients were grouped into tertiles according to their resistin level.Carotid IMT was significantly highest in the third tertile. The first tertile had a mean carotid IMT and a mean of maximum carotid IMT of 0.61±0.11mm and 1.17±0.21 mm, respectively. The 2nd tertile had measurements of 0.64±0.11mm and 1.20±0.25 mm, and the 3rd tertile 0.71±0.14mm and 1.33±0.26mm for the same parameters(FIMT=4.017,PIMT=0.021; FmaxIMT=3.387,PmaxIMT=0.039). Resistin levels were independently associated with the carotid IMT (carotid IMT: R2=0.432, F=62.609, P=0.000 and maximum carotid IMT: R2=0.385, F=40.343,P=0.000) after controlling for age, blood glucose, uric acid, body mass index(BMI), total cholesterol(TC) and diabetes mellitus.3,Another grouping method, all hypertensive patients were grouped into tertiles according to their leptin level.Carotid IMT was significantly highest in the third tertile. The first tertile had a mean carotid IMT and a mean of maximum carotid IMT of 0.59±0.10mm and 1.07±0.27mm, respectively. The 2nd tertile had measurements of 0.65±0.10mm and 1.20±0.19 mm, and the 3rd tertile 0.70±0.17mm and 1.31±0.22mm for the same parameters(FIMT=6.805,PIMT=0.002;FmaxIMT=10.744,PmaxIMT=0.000).Leptin levels were independently associated with the carotid IMT (carotid IMT: R2=0.303, F=7.766, P=0.000 and maximum carotid IMT: R2=0.270, F=14.438,P=0.000) after controlling for age, blood glucose, TC and high density lipoprotein-cholesterol(HDL-C).Conclusion:1,Serum resistin,leptin and carotid IMT were significantly higher in the hypertension patients than the healthy controls respectively; serum resistin,leptin are associated with increasing carotid IMT in essential hypertensive patients respectively.2,Serum resistin is independently associated with increasing carotid IMT in hypertensive patients.3,Serum leptin is independently associated with increasing carotid IMT in hypertensive patients.4,The study has shown a potential role of resistin,leptin in the pathogenesis of atherosclerosis in hypertensive subjects.So as to contribute to the prevention and treatment about atherosclerosis in hypertensive subjects.
