| Background: The population aging is inevitabled due to the deline of the birthrate and mortality, extension of mankind's life expectancy and the social development. From the viewpoint of the whole world, now there are many developing countries which have already stepped or are stepping into the elderly society. According to UN statistics, China has already entered into the elderly society in 1999, and the population of the elderly has been near or over 10%. The absolute number of the elderly in China is not only in the first place in the world, but also one of the quickest nations of aging. The population of the aged in China will be up to 12.6% in 2010. Jilin province has entered into the aged society early in 2003, Lishu is a one county of Siping city. Census statistics by the end of 2007 indicated that the total population of LiShu is 7 866 000, the old people over 60 is 814 000. And the rate of the old is 10.34%. Show that the county of Lishu has also entered into the aged; however the prolonging of expected life-span doesn't mean the healthy aging. As the age increased, health status and ability of activities of daily living (ADL) of the elderly are decreasing; chronic disease and disability are increasing. Therefore, the community health service should be greatly developed, and it is the primary task for the community health personnel to improve the health quality of the old.Objective: To provide predictive and decisive basis for the old people preventing from diseases and community health service work, through the survey of chronic diseases prevalence and health service needs of the old population in the suburb and rural areas of LiShu to evaluate their health quality.Methods: 821 old people over 60 in the urban and rural areas of LiShu were surveyed as the target, using the combined method of stratified sampling and systematic sampling. Adopt the questionnaire of"health state and the demand for health services of the elderly"to check for the old people by the trained investigators. The data were inputed into the database, which was set up by the means of Epidata 3.0 software and analyzed with SPSS 13.0 software. Measurement data and numeration data were compared with t test andχ2 tests respectively.Results: The results showed that there were more female than male, male to female ratio was 0.45:1. And the average age of females is elder than the male. The knowledge level and income of the old people is low. The total prevalence rate of choric disease was 90.6%. And 67.5% of the eldly suffered from more than two kinds of chronic diseases. The top five ranks in chronic diseases were heart disease, hypertension, diabetes mellitus, osteoarthrosis disease, Cerebrovascular Disease. Besides Hyperlipidemia is Widespread in the old, the rate up to 67.5%. Demands for health services were high among the elderly; But over half of them have't go to the major hospitals because of financial difficulties. The proportion of the selection to health education only was 5.0%.Conclusion:1. The knowledge level of the old people is low, and lacked of consciousness of prevention and health care. Most of the rural elderly were agriculture worker, and no pension, so they are very necessary to health service.2. High prevalence rate and prevalence of various types of chrnic diseases, and the Cardiovascular disease is major problem which affecting the elderly health. Healths Behavior Situation of the elderly are poor .The dangerous factors to CNCDS exist in common.3. Demands for health services were high among the elderly. The concept of prevention disease and health education are backward, in the selection of health service project, the proportion of health education only was 5.0%.4. Financial difficulties are the leading factor to affect the urban or rural elderly'selection of health providers in Lishu County.
