| World Health Organization (WHO) claims that countries all over the world should develop community health service vigorously, and make it become an important method and approach of promoting the primary health care in order to achieve the goal of"21 century everybody enjoys health", that is: enable all the people to aggrandize anticipated lifetime and elevate quality of life, accelerate health equitableness between and in the countries, and enable all the people to gain continuable health service. Community health service (CHS) was evoluted in the Europe and America developed countries in the 1960s, and now has been recognized as a best basis medical mode by all the countries. It provides a integrated service that is facing to community, according to unit of family and person-centered. And develop CHS is a strategy act in the process of our country's health innovation and course, and important phase and element of medication system for cities and towns.CHS is a community directed health service that engaged by health organization or institution and general practitioner (GP) as the main body. The community directed health service way is different from the speciality service way in the hospitals, it is under the government leading, community engaged, and authorities entity guiding, using basis health entity as main body, and can use the health resource and amenity technique advisablely. Its center is health, unit is family, scope is community, guidance is requirement, and emphasis is female, children, old people, people who has chronic disease and disability, and fragile persons, and it in order to solve the main health questions and fill the basic requirement. CHS contains prevention, medical treatment, health care, education, and birth control, can provide effective, economic, general, and continuous basis health services.In order to search demography characters, awareness condition of health knowledge, behavior way, health service understanding and utilization, andhealth condition and its effect factors, analyze and propose the questions that should be solved firstly, and provide reference accordance for government and other related social departments about establishing health planning, deploying resources reasonable, and making scientific decision.The sample is gained by the stratified sampling method and systematic sampling method. The study methods include references, describe and factor analysis method, SPSS12.0 was used to analyzed the data and the effect of each quota, main analysis methods are chi-square test, nonparametric test, and logistic regression. All the survey staffs were trained strictly, and accessed eligibility. Divide the staffs into groups according to the survey projects, and they used unified questionnaires and standards, explained meanings to the respondents according to the informed agreement principle. The staffs filled in the questionnaires, checking staffs checked them and their accuracy and integrality, if find mistakes or leak writings, should feed back to the survey staffs, and add or correct in the next day. If the body examination result was unfilled or big discrepancy, survey staffs should call the objects for another examination. All the data were logged in EPIDATA2.0 data base, two groups of log-in staffs logged in the data differently, and compared after finish in order to ensure its accuracy. The data were real, reliable, complete and accurate.The study investigated 1000 families, 2387 persons were investigation objects, of which male 1161, and female 1217, and male and female gender contrast was 95.4. Young people group, middle age group, and old age group were each account for 29.94%, 52.57%, 17.49%; nation mainly was Han; graduated from senior high school and junior high school, and married, workers, retirees were the most people. The first five chronic diseases were heart disease(6.06%), hypertension(5.80%), diabetes mellitus(3.03%), cerebral vascular disease(1.77%), and rheumatism(0.55%). It suggests that communities in the main control chronic heart disease, hypertension, diabetes and cerebrovascular disease prevention, focus on intervention and the starting point.Chronic disease was the important effect factor of inhabitant health and their quality of life, and when the prevalence was lower, the quality of life of who has chronic disease can improve. It is very important to guide resident form good life custom, adopt correct behavior, have a fulfill cognition of risk factors. Through the whole people health educational exercise can provide more knowledge about disease prevention and avoid risk factors.In conclusion, it is very important to carry on CHS all round, it could accelerate the development of economy level, improve the people situation and social spiritual civilization, and solve the problem of no-easy to see the doctor. CHS has the enormous function.
