?Objectives?This study will further clarify the critical care problems of post-acute stroke older patients.The aim is to form a key index system to help the nurse identify,evaluate and obtain the key health problems of post-acute stroke older patients,formulate appropriate and effective nursing plans to meet the care needs of this population.?Methods?1.On the basis of literature research,current situation investigation and qualitative interview,using Maslow's hierarchy of needs and Orem self-care theory as theory framework,the key index system of nursing assessment for post-acute stroke patients is preliminarily formulated.2.The preliminary evaluation of the key index system of nursing assessment for the post-acute stroke patients was carried out in two rounds of Delphi expert consultation and weight setting,and the key index system was finally established.?Results?1.Current status survey results The questionnaire made by research team was used to investigate 287 older patients with stroke and 247 non-stroke older patients.The incidence of common nursing problems in the post-acute period was compared between the two groups of older patients to screen out the key indexes of nursing assessment for older stroke patients.Finally a total of 3 first-level indexes and 14second-level indexes were selected.2.Qualitative interview results 8 stroke care nurses were interviewed by semi-structured interviews using the outline discussed by the research team,and further supplement 6 secondary indexes.3.The results of Delphi expert consultation The 3 primary indexes and 20 secondary indexes formed by clinical research and qualitative interviews were consulted by two rounds of experts.After two rounds of expert consultation,the positive coefficient of the expert is(85.71%,91.67%);the authority coefficient of the expert is(0.78 and 0.79)respectively;the variation coefficient was between 0% ~13.7%;the coefficient of Kendall harmony of each indicator was in 0.4 above,and the thechi-square P value is less than 0.05.According to the recommendations of the experts,3 first-level indexes,16 second-level indexes and corresponding evaluation methods were established.The three primary indexes cover physiological status,mental state and functional status,facilitating clinical care nurses to identify,assess,and obtain critical care problems from three levels,and develop active and effective care plans for patients.?Conclusion?Based on literature research,current situation investigation and qualitative interview,combined with Delphi expert consultation and AHP,the key indexes of nursing assessment for older post-acute stroke adults are scientific and reliable,which facilitate nurses to identify,evaluate and obtain the patient's critical care problems.In addition,the key indexes can provide a basis for construction of referral criteria,the level of care needs,and the development of key care assessment indicators for other geriatric diseases.
