Objective To study the clinical manifestation,diagnosis and treatment of primary gastric lymphoma(PGL).Methods The data of all 26 patients over the recent 5 years(2003-2007)in the series with PGL were retrospectively reviewed.Patients were eligible if they had biopsy-confirmed diagnosis obtained by endoscopy or following operation.Clinical and endoscopic features were analyzed,and histologic examination was determined.The survival rate is analyzed by Kaplan-Meier.Result(1)the upper abdominal pain,intestinal bleeding,and weight loss were common.(2)2 cases(8.3%,2/24)were misdiagnosed as gastric ulcer or cancer under gastroscopy.(3)22 cases underwent operation,among them 16 had been performed a radical operation,3 cases underwent palliative operation,3 cases underwent exploratory operation.2 cases only received chemotherapy,and 2 cases received chop chemotherapy and anti-infection of HP treat.(4)In 20 cases receired 6-month to 60-month following up,the 3-year survival rate was 70.9%,the 5-year survival rate was 56.7%.The survival rate of PGL of low grade and early stages(â… E andâ…¡E)was better than those of high grade and late stage ones(â…¢andâ…£).Conclusion There are no specific symptoms in gastric lymphoma patients,so the misdiagnosis of PGL is frequency.Preoperative diagnosis depends mainly on gastroscopy with biopsy.The multiple biopsy sampling from submucosal layer may improve diagnostic rate on primary gastric lymphoma.Radical resection is the key factor in improving the prognosis of PGL patients,Chemotherapy,radiotherapy,anti-infection of HP are useful adjuvant treatments.The survival rate has a significance relationship with the depth of infiltration,the grade of pathology and the stage of PGL.
