Cerebral palsy(CP)describes "a group of permanent disorders of the development of movement and posture,causing activity limitation,that are attributed to nonprogressive disturbances that occurred in the developing fetal or infant brain.The motor disorders of cerebral palsy are often accompanied by disturbances of sensation,perception,cognition, communication,and behaviour,by epilepsy,and by secondary musculoskeletal problems." It remains a major cause of chronic childhood disability and is becoming an important public health problem in the world because it affects physical and psychological health of children.At present,cerebral palsy is focused on in the world.The goal for these children, their families,medical care,education,and society at large is for them to grow and develop to their maximum capabilities so that they may succeed as contributing members of society and independence.The way to organize the management of these well child care needs is with a multidisciplinary clinic in which a primary care pediatrician is present.In the paper,firstly,we reviewed the knowledge of cerebral palsy on the name and the etiology and pathogenesis in traditional Chinese medicine,summarized all kinds of acupuncture and moxibustion techniques and the mechanism in the recent two decades.We also understood the knowledge of CP about the etiology,classification,auxiliary examination,treatment,and evaluation in modern medicine.However,the understanding had been still continuously intensified and perfect.Then,we reviewed the history of music therapy performed by means of magic and religion and changes into a systematic treatment,which revealed music contribution to our survival.The literature proved that the methods of music therapy becomes more and more, and it was continuously applied to a wide range.But,most trials were small models,lack of convincingness,and the mechanism needed to be deeply understood.The paper aimed to study the effects of the cooperation of acupuncture and music therapy on the treatment of cerebral palsy.METHODS:all the cases in the trial were from the outpatient department in Shenzhen hospital of Traditional Chinese Medicine during the period from July 1st,2007 to March 15th,2008.On the premise that all children's patients or guardians knew the study and agreed to receive it,a randomized controlled was conducted to the patients who met the western internalizing and depleting standards.All 40 patients were randomly allocated to two groups,namely 20 Children in Group A were given acupuncture and music therapy while 20 patients in Group B were given only the acupuncture therapy.The basic acupoints were essential to all participants,selected according to the theory of the relations among brain,Zangfu and meridians,and developmental behavior,including Sishenzhen,Niesanzhen,Naosanzhen,Zhisanzhen,Shousanzhen,Shouzhizhen,Zuzhizhen, and Zusanzhen.The individuals were added acupoints for different symptoms and syndromes.At first,flashly needled acupoints for syndromes for 0.3~0.1 cun.For other acupoints,the technique of transverse needling was applied to acupoints in the head,and the perpendicular needling was to other ones.Twirled and inserted needles and then kept for 30 min.Twirled the needle for ls every other 10 min without reinforcing-reducing techniques.The therapy was done every 2 days.The trial had 2 periods each of which lasted 84 days.Subjects in Group A were given music therapy.They listened the Wuxing music during the time of acupuncture.After acupuncture,they had music activities like beating instrument,singing and dancing.The music therapy was scheduled for 1h including listening music for 30min and music activities for 30min.The comprehensive functional evaluation of cerebral palsy was applied to test the symptoms before and after respective treatment.The mYPAS were tested children's anxiety before and at 1min,15min,30min after acupuncture.The CHEOPS and FACES were tested children's pain before and at 1min,15min,30min after acupuncture.RESULTS:1)the basic data in two groups before the treatment were closely similar in all important prognostic variables includes age,sex,classification,etc.(P>0.05).2)The scores of mYPAS increased from baseline to lmin,decreased among two groups at 15min,especially Group A decreased significantly,and then increased little at 30min(F=6.60,P=0.014<0.05).There was no difference between two groups before and at 1 min(P>0.05),but difference at 15min and 30min(P<0.05 or P<0.01).3)All patients were with less than 6 scores of CHEOPS and FACES before acupuncture.The patients with less than 6 scores of CHEOPS in Group A were more than Group B at 1min,but the FACES was reversed.The patients with less than 6 scores of CHEOPS and FACES in Group A were more than Group B at 15min and 30min.4)For the comprehensive functional evaluation,the total rate of effects in Group A was higher than Group B,with no significant difference(P>0.05).The patients in Group A were relieved in functions of cognition,speech,movement and social internation(P<0.05), while only the cognitive and movement functions in Group B(P>0.05).But the self-care function in two groups were not relieved.There was significant difference betweet two groups in the functions of cognition and social internation(P>0.05).CONCLUSION:1)It is feasible and effective to treat CP with acupuncture according to the theory of the relation among brain,zangfu and meridians,developmental behavior.2)The music therapy can reduce anxiety and pain in children with cerebral palsy during acupuncture,and improve the quality of life.3)It is feasible to treat CP with music therapy,and it is more effective with the cooperation of acupuncture and music therapy on the treatment of cerebral palsy,especially in the functions of cognition,speech,movement,and social internation.
