Vascular Cognitive Impairment (VCI), of cerebral vascular diseases caused by all the different levels of intelligence diminish, is the clinical syndrome. VCI includes vascular dementia, a vascular component of Alzheimer's disease (AD) and not meet the diagnostic criteria for vascular dementia and cognitive impairment. Vascular cognitive impairment in patients is relatively acute onset, but also for many times the incidence stable again after a sudden increase, showing progress or ladder-like volatility, and 20 percent to 40 percent of cases onset hidden passage, with continued progress. The mechains of VCI is related to excitatory amino acid toxicity doctrine, inflammatory theory, free radical theory, theory of lack of acetylcholine, however, its specific mechanism is not yet very clear.Chinese Medicine for VCI spent a long history of understanding. Through the cause of VCI awareness of the clinical practice and research, it has accumulated rich experience in herbal therapy, acupuncture rehabilitation, health care. Vascular cognitive impairment is a chronic disease. Points catgut embedding is the product of the combination of meridian law theory and modern medical therapy. It achieve the purpose of treating disease, through the function of catgut in points in the physiological role of physical and biochemical changes in the information and energy through stimulating Meridian into the body. To the whole treatment process, including the effect of a needle to stimulate the thorns and bleeding thread embedding blood effects, and the thread-point in the long-term stimulus, it reaches to adjust the normal function of zang fu organ and to cure diseases. It is also in line with the point of view that 'deep long term treatment to cure disease' in Huang Di Nei Jing(Canon of medicine) of view.Object: To explore the effect on treating vascular cognitive impairment through the back-shu point catgut embedding.Methods: Vascular cognitive impairment in patients with 63 cases were randomly divided into groups back-shu point catgut embedding31cases, 32cases of conventional acupuncture group. Back- shu points catgut embedding group using shen shu (BL23), xin shu (BL15), jue yin shu (BL14), and ge Shu (BL17). The conventional acupuncture group using bai hui(GV20), si shen cong(EX-HNl) ,tai xi(KI3), da zhong(KI4), xuan zhong(GB39), zu san li(ST36). There is a total of two courses, for 20 days each course of treatment. The patients are valuated by MMSE, ADL, CSS and Chinese symptom measuring scale before and after the two cause's treatment.Results:1. Clinical Evaluation of standards under the MMSE scale back-shu point catgut embedding group of 80.6 percent efficiently, conventional acupuncture group overall response rate was 78.1 percent, the Ridit analysis, the overall effect of the two groups no significant difference (P > 0.5), This shows that the clinical effect of using back-shu-point catgut embedding and conventional acupuncture methods improving patients with cognitive impairment is significant.2. MMSE inventory data show that before treatment back-shu point catgut embedding group 13.77±4.50, after treatment for 16.32±5.38, conventional acupuncture group before treatment 13.78±4.47, after treatment 15.04±4.77, the two groups before and after treatment by the t-test, a significant clinical effect (P <0.01), comparison two groups, there is no statistical significance (P>0.05). Note the improvement in the direction of computing power, attention, calculation, and memories of areas such as language and catgut embedding and conventional acupuncture points are an advantage.3. The evaluation of ADL scale back-shu point catgut embedding group before treatment 49.84±13.99, after treatment 56.12±15.69; conventional acupuncture group before treatment51.20±16.22, after treatment 56.61±15.52. The two groups before and after treatment group were the t-test, the effect of significantly (P<0.01); Comparison two groups, there is no statistical significance (P>0.05). Note back-shu point catgut embedding and conventional acupuncture in improving the daily lives of patients such as the ability to eat, from top to bottom stairs, dressing, bathing, and 10 aspects of it have a good clinical effect.4. The evaluation of CSS scale back-shu point catgut embedding group before treatment 18.84±8.13, after treatment 13.16±4.80, conventional acupuncture group before treatment 19.92±7.94, after treatment 15.93±5.30. The two groups before and after the treatment group by t test, P <0.01; comparison group, P <0.05, note back-shu point catgut embedding group for the improvement in patients with symptoms of neurological deficit has advantages.5. According to TCM-scale assessment shows that: before and after the treatment group by t test, back-shu point catgut embedding group on syndrome of obstruction of collaterals by blood stasis, the effect is significant (P <0.01); syndrome of kidney essence insufficient, syndrome of qi and blood deficiency, syndrome of obstacle fu organ ,syndrome of turbid phlegm blocking the clear orifices, syndrome of obstruction of collaterals by blood stasis , syndrome of excessive rising of liver-yang, syndrome of dominance of heat in the interior, such as various types of treatment and there is significant difference (P <0.05); conventional acupuncture group on syndrome of obstruction of collaterals by blood stasis and dominance of heat in the interior , there is significant difference (P <0.01), the remaining before and after treatment there were significant differences (P < 0.05). Comparing group, syndrome of excessive rising of liver-yang and syndrome of obstacle fu organ by t-test, back-shu point catgut embedding has a significant effect (P <0.05), all of the remaining P> 0.05. Back-shu point catgut embedding has advantage on threating the syndrome of obstruction of collaterals and syndrome of dominance of heat in the interior patients with vascular cognitive impairment.Conclusion:The overall effect on treating vascular cognitive impairment using back-shu point catgut embedding and conventional acupuncture is same. Back-shu point catgut embedding in improving neurological function in patients with the defect has advantages, and back-shu point catgut embedding method is simple, save time, in the promotion of a better degrees.
