Background Organism could display the stress reaction of autoprotection and up-regulated selectivitily the expression of heat-shock protein70 acted as molecular chaperones,which protected apparently the infected cells,when it was infected by virus.Hemorrhagic fever with renal syndrome was an acute infectious disease caused by Hantavirus which major clinical feature was composed of fever and haemorrhagia and hypotension shock and renal failure.It was considered now, vascular endothelial cell was a primitive target cell about Hantavirus and circular endothelial cells were the defluviumed vascular endothelial cell which acquired from the circulation of blood.Nuntil now,whether the vascular endothelial cell in patients with HFRS could generate stress reaction and produced the expression of HSP70? whether HSP70 could protect vascular endothelial cell from damage? It was not reported well-definedly.Object To study the significance of the changes of circular endothelial cells(CEC)and the ecpression of heat-shock protein70(HSP70)in patients of hemorrhagic fever with renal syndrome(HFRS),and study the mechanism of HFRS.Methods Fifty-for HFRS patients were assigned to mild group(20 cases), moderate group(20 cases)and severe group(14 cases)according to their illness condition,and 20 healthy volunteers were selected as the controls.After the CEC were separated by using isopyknic centrifugation method,The CEC number and the HSP70 level were determined with flow cytometry to observe the changes of CEC number and HSP70 in patients with different stage and type of hemorrhagic fever with renal syndrome.Results In patients with different stage of HFRS,the number of CEC were statistically higher than those in the controls,It began to increase in the fever stage and maintain a high level in the hypotension and oliguria stage,then decreased gradually in diuretic phase and recovery phase, The curves of them were similar to those of BUN,but were contrary to platelet counts;There was similar change in the expression of HSP70 In patients with different stage of HFRS.The expression of HSP70 is not significantly relative with the number of CEC in the fever stage(r=0.201 P>0.05),but it is positively relative with the number of CEC in the hypotension and oliguria stage(r=0.835 P<0.05).In patients with different type of HFRS,As the condition of these patients was aggravated, The CEC number and the HSP70 level increased accordingly and reached the peak in severe and danger HFRS.In patients with different stage and type of HFRS,the number of CEC and the expression of HSP70 were both significantly higher than that of controls.Conclusion The CEC number and the expression of HSP70 in CEC both increase in the HFRS patients and the increase extent is related with the stage and linical type of HFRS;They increased significantly and were positively related with renal injury。There found no positive relationship between The CEC number and the expression of HSP70 of HFRS patients in fever stage while in low blood pressure and shock phase significantly positive relationship were found.The expression of HSP70 could help to increase the defence funtion of organism and protect vascular endothelial cells.
