Objective:To investigate the change of extracellular (serum) heat shock protein70(eHSP70) in patients with MODS dynamicaly, explore the association of eHSP70expression with inflammatory facors and the relationship with the disease severity, as well as examine the clinical value of eHSP70while estimating the outcomes of MODS patients.Methods:80patients with MODS aged18to89years were collected from ICU admission and another80normal persons aged22to77years as control group. The levels of eHSP70on admission and in the24hours after hospitalization were determined by ELISA respectively. Compute the change of eHSP70within24hours and also measure the inflammatory facors. According to the survival conditions after20days treatment, the cases were classified as "survival group" and "death group".The change of eHSP70were compared in this two groups and analysed by Logistic regression analysis.Results:The levels of eHSP70at different times in MODS patients were significantly higher than those in control group (P<0.05). Compared with the survival group, the death group of MODS patients showed an evident increased level of eHSP70. eHSP70has a positive correlation with inflammatory factors. Adopting Logistic regression analysis, APACHE Ⅱ scores, the number of dysfunctional organs and especially the change of eHSP70were proved to be independent and reliable risk factors for estimating the prognosis in MODS patients.Conclusions:The levels of eHSP70on admission, in the24hours of after hospitalization as well as the change of eHSP70within24hours are all closely related with the disease severity and inflammatory factors. And the change of eHSP70is an independent predictor for the prognosis in the MODS patients.Significance:Dynamic monitoring of serum HSP70levels and the changes in the different periods of different prognosis in patients with MODS, which combined with the APACHE Ⅱ score, we analyze the relationship between serum HSP70and disease severity of MODS in patients and its prognostic value, and explore its clinical significance in the treatment of MODS.
