Objective:To explore the prevalence and pathogenetic features of knee osteoarthritis among the employees over 40 years in Tianjin,so as to provide some available evidence for precaution and treatment of osteoarthritis of the knee.Methods:From April to July 2007,those employees over 40 years in Tianjin were surveyed.Protocol of APLA-COPCORD Core Questionnaire for identification of risk factors for knee OA was implemented.Physical examination was carried out among the subjects who had knee pain and were given both knees normotopia and lateral position radiographic examination.Each person was measured stature,avoirdupois, blood pressure,and was tested blood routine,glycemia,glycerin,total cholesterin, uric acid and creatin.Diagnostic criterion of knee osteoarthritis was constituted with ACR in 1995.Variables such as sex,age,body mass index(BMI),smoking,drinking, working posture,the manner of go to work,the habit of eating,glycemia,glycerin, total cholesterin,uric acid and creatin,were included in binary logistic regression model for further analysis.Results:①Prevalence rate of knee pain and knee OA was 15.5%and 14.0% respectively,significantly higher then in Shantou-in south of China.②The prevalence of knee OA of men was 9.1%,the prevalence of knee OA of women was 19.4%.Both of these prevalence was significantly higher in women than that in men(x~2=49.602, P<0.001),and with a tendency of being increased with age.③BMI in knee OA group (25.3±3.4kg/m~2)was significantly higher than than in non-knee OA group (23.9±2.8kg/m~2),it existed statistical difference(t=7.944,p<0.001).In women, with increasing of BMI,the prevalence of knee OA was significantly increased.But this change was not been observed in men.④Prevalence rate of knee OA in menopause women was significantly higher than that in non-menopause women (23.8%比8.5%,x~2=34.88,P<0.001).Menopause age in women knee OA group (50.3±3.8)was younger than that in women non-knee OA group(52.7±4.1),it cxisted statistical difference(t=7.665,p<0.001).The earlier the menopause,the easer the osteoarthritis appeared.⑤Binary logistic regression revealed that age,sex and BMI were significantly with knee OA.And eating vegetable could prevent the osteoarthritis.Whereas there were no significantly association between the other factors such as smoking,drinking,working posture,the manner of go to work.Conclusion:Prevalence rate of knee OA was 14.0%.The prevalence of knee OA of men was 9.1%,the prevalence of knee OA of women was 19.4%,both of these prevalence was significantly higher in women than that in men.Old age,female,and overweight might be the associated risk factors of knee OA.No association between smoking,drinking,working posture,the manner of go to work and knee OA was found.Prevention of knee OA should be initiated before the middle-aged,especially in female.Controlling body weight,avoiding obesity and correct physical exercise are important in preventing knee OA.
