A Clinical Study On The Treatment Of Active Lupus Nephritis With The Method Of Yin-Nourishing, Detoxifying And Promoting Blood Flow (YNDPBF)

Objective:Lupus Nephritis(LN) has been the most important reason of secondum damage on renal function in China. This 20 years the prevalence of LN is increasing year by year, and its lethality is very high, so researchers pay more attention on it. Up to now, LN is still an uncurable disease. The side-effect of Western medicine is very serious, and it recurs easily. The objective of this study is to compare the clinical curative effect of Integrated Traditional Chinese and Western Medicine (ITCWM) on LN with simple Western Medicine(WM). To observe the side-effect and syndrome between ITCWM and WM. To prove that Traditional Chinese Medicine(TCM) can add curative effect and reduce poisonous or side effect in treating LN. And then set up a safer, more effective treatment of ITCWM, which can be used for clinic.Methods:A total of eighty LN patients were divided randomly into two groups, the treated group (n=40) and control group (n=40). The control group was treated mainly with conventional WM (Prednisone(P) and Compound Cyclophosphamide(CTX)), while the treated group was treated with ZhiBaiDiHuang Recipe (ZBDH) on the basis of conventional WM. Both groups were kept on treating for 3 months. The main clinical symptom and objective sign were observed, and then fill in the tables of SLEDAI and SLEFI. The lab's indexes such as WBC, HGB, PLT, PRO, BLD, BUN, Cr, ESR, CRP, C3, ANA, Anti-ds-DNA and 24bunp were compared and summarized. The dosage of prednisone, occurrence ratios of side effect were added up. Results:Treated for three months, On the aspect of clinical curative effect, the total effective ratio in the treated group(95.0%)was significantly higher than that in the control group(82.5%), the difference had statistical significant(P<0.05). On the aspect of TCM symptom, such as fever, erythema, hair lost, mouth ulcer, arthralgia, both of the groups improved them. And the total effective ratio in the treated group(97.5%) was obviously higher than that in the control group(82.5%), the difference had statistical significant(P< 0.01). The level of SLEDAI and SLEFI in both groups were decreased, and the treated group was obviously lower than those in the control group, the difference had statistical significant (P< 0.05). The lab's indexes such as ESR, CRP, Upro, Urbc, Cr, C3, Anti-ds-DNA and 24 hours upro in two groups were obviously improved.Further more, the difference between the treated group and the control group had statistical significant (P< 0.05). On the aspect of cutting down the prednisone, the daily dosage of prednisone at the ninetieth day in the treated group was obviously lower than that in the control group (P< 0. 05), so was the total dosage of prednisone during the 3 months (P<0.01). The occurrence ratios of side effect such as infection by bacterium or virus, digestion dysfunction, hypertension, hyperblood-sugar, hyperblood-lipoprotein, insomnia or inordinate menses in the treated group were obviously lower than those in the control group.Conclusions:The results of this study indicate that treating LN with the method of YNDPBF combining with WM is more effective than simple WM. ITCWM can effectively increase the clinical curative effect, improve the clinical symptom and lab's indexes, reduce the dosage of prednisone, benefit for cutting down prednisone, and significantly decrease the occurrence ratios of all kinds side—effects.
