The Treatment Strategy Of Radial Head Fractures In Adults

Objective In order to discuss the treatment strategy of the adult radial head fractures,we choose some patients to make a follow-up visit,and we carried on the review analysis to evaluate the curative effect with Broberg and Morrey score system. We expect that it can be a reference for clinical therapy.Methods 27 cases of radial head fractures were accepted to our department from 2001 to 2007,but 4 cases were lost.Then it remained 23 cases which has the whole clinical data,among them,6 cases received conservative treatment(all type Masonâ… ),11 cases received ORIF(all type Masonâ…¡),6 cases received radial head excision treatment(5 type Masonâ…¢,1 type Masonâ…¡).Mason classification and Broberg and Morrey score system were used to analyze the correlation between the results and the factors of fracture types and surgery methods.Results After the treatment each group was followed-up for 1.0-6.5 years, the average 3.8 years. Evaluation was done according to the Broberg and Morrey score system .The excellent and good rate of treatment in nonoperative,ORIF as well as excision of radial head was 83.3% (5/6),90.9% (10/11),66.7%(4/6)respectively,comparing with a group of domestic clinical data and two groups of overseas data 84.8%(P>0.05),88%(P>0.05),73.3%(P>0.05) respectively.Conclusion We should choose the treatment of the adults radial head fractures according to the classification,and also concern the degree of the injury,such as ligaments'injury, parenchyma injury and so on.The type Masonâ… and type Masonâ…¡fractures can be carried on the conservative treatment and ORIF treatment respectively. Radial head fracture should be anatomicaly reducted as far as possible, if can not be reconstructed,the radial head excision or mental prosthesis replacement is alternative.
