The Research Of Hospital Developmental Strategy And Market Status Of Medical Care Service In Hengyang City

Objective. The aim of this paper is realizing the status quo of the medical care service market, and analyzing the problems of medical care resource allocation, and medical care service supplying, and the burden of medical care costs, and the situation of inhabitants' medicine care payment, and the administration of the hospital. It also would discuss the reasons of these problems and explore the ways about how to solve them, which could provide references and basis for the health administrative departments of the city to design scientific health policies and hospitals of the city to design developmental strategy in Hengyang.Methods: Using the technology of data warehouse and statistical description, this paper has established a data frame on the basis of multidimensional data aggregate of the information during 2004-2005 of all the hospitals and commune health stations in Hengyang, which included the number of beds and doctors of hospital, and person-times for care, person-times and income of outpatients, person-times and income of inpatients of hospital, and income of drugs; then it also would carry out the statistical description of hospital, analyzed and research through five aspects: basic situation, resource allocation, medical care service supply, the situation of people for medical care and the status of hospital management.Results: The number of medical care institutions and the number of beds per thousand people is obviously higher in the Hengyang city than in the county; the beds in the grade AA and AAA hospitals is nearly 60% of the whole beds of hospital in the city. So the medical care resource allocations are imbalanced between the city and the county. The person-times of diagnosis and treatment, person-times of inpatients, and the times of operations per year in the grade AAA hospitals account for a great proportions, which have nearly a half of business income in all the hospitals in the city. And the business income from the grade AA hospitals has occupied the second biggest propotion. The rates of person-times of outpatients and inpatients per thousand people in Hengyang are under the national average level. The average medical care cost of outpatient and inpatient per time increases along with the heightening level of the hospitals; the cost for medical care in hospitals per time in city account for 43% of the peasants' net income per person for a whole year.The material medica care of hospitals in Henghang has a profit. The profit margin from medicine in the grade AAA hospitals is the highest while the medicine treatment is in a loss. The income and expenses of business in the grade AAA hospitals has a surplus while the other levels of hospitals have different deficit.Conclusions: 1. The medical care resource allocation is unreasonable in Hengyang, including the level of the resource allocation in city is higher than that in country, the level of big hospitals is higher than that of subordinate hospitals, and the differences of the medical care resource allocation in each county are remarkable. 2. The medical care resource supply can basically satisfy the demand in Hengyang. The ability of medical care service supply is stronger in big hospitals, and the hospitals in Hengyang are developing to two extreme directions.3. The burden of medical care costs is heavy for people, and the medical care cost payment is very high. 4. The phenomenon of "medicine care maintenance by drugs" is very remarkable.Suggestion: 1. We should carry out the district health plan, and optimize the allocation of medical care resources. 2.We should strengthen the construction of community and country medical care services systems. 3. We should enforce the supervision of medical care institutions, standardize the medical cost charge, reduce the drugs' prices, and decrease the medical care cost burden of the patients.4.We should make the principle of referral more perfect, enlarge the charge standards among the hospitals of different levels reasonably, and guide the patients visit doctors to flow to different hospitals correctly. 5. We should carry out the health care financing to the basic service items of medical care institutions, reform the management system of hospital and adopt a more flexible management policies of the medical care service prices.6.We should adjust the medical care service market by means of "plan" and "market". All levels of the hospitals should identify their market location well, improve the hospital management efficiency, change the extensive type of evolutive mode, and quicken the development for the aim of intensive type of development.
