| The first departmentObjective: To study the therapeutic effect of Jiedu Huoxue Prescription on Peptic Ucler.Method: The case-control study was adopted. The selected cases according to"Randomized Number Scale"were randomly divided into two groups: 35 patients in treatment group were administered with Jiedu Huoxue Prescription; 35 patients in control group were administered with Ranitidine. Both groups were treated for four weeks. Clinical symptoms'changes, gastroscopy changes, effect on Helicobacter pylori (Hp) before and after treatment were detected respectively to evaluate the therapeutic effects.Result:(1)Clinical symptom therapeutic effect: In treatment group the total efficiency is 85.70%,which is significantly higher than that of control group(P<0.05).In control group the total efficiency is 74.30%. (2) Gastroscopic therapeutic effect: In treat- ment group the total efficiency is 91.40%,which is significantly higher than that of control group(P<0.05).In control group the total efficiency is 63.33%.(3)Effect on Hp: In treatment group Hp positive patients were 29 before treatment, the patients were 10 after treatment; the negative is 65.52%, which is significantly higher than that of control group(P<0.05). In control group Hp positive patients were 28 before treatment, the patients were 17 after treatment; the negative is 39.30%.(4) Compared the main clinical symptom before and after treatment: All the main clinical symptoms were markedly improved after treatment, while only gastric abscess and sour regurgitation were improved after treatment in control group. There was a significantly difference between treatment group and control group after treatment in dry mouth and constipation. (5)Compared ulcer prostecdtive efficacy: the ulcer recurrence rate in treatment group was 9.1% while in control group was 40.0%.There was a significantly difference between two groups.(6)Evaluate therapeutic safety in the two groups: There were neither obvious changes in blood, urine or stool rules before and after treatment nor in Electrocardiogram or liver-renal function. In the course of the treatment, all patients had no undesirable response, which pointed the fact that Jiedu Huoxue Prescription has no toxic and side effect.Conclusion: The study indicates that Jiedu Huoxue Prescripti- on can improve patient's clinical symptom, restrain or eliminate Hp ,improve quality of ulcer healing to decrease the recurrence rate of Peptic Ulcer. Its curative effect is better than that of Ranitidine. The research from clinical curative effective aspect approves that Jiedu Huoxue Prescription is feasible and practicable in treating Peptic Ulcer.The Second DepartmentObjective: To explore mechanism of Jiedu Huoxue prescripti- on in treating gastric ulcer by constructing animal model and obser- ving correlated indexes. The aim is to provide experimental eviden- ce for clinical research.Method: 40 healthy male SD rats were selected and randomly divided into five groups, each group had 8 rats. Rat models of gas- tric ulcer were established by hawkinsin. 24 hours after the opera- tion, the amount of dried medicinal herb given to large dose group and low dose group was 63.4g/kg weight and 31.7g/kg weight respectively. The amount of ranitidine given to ranitidine group was 2.7mg/100g weight. The normal saline was given to normal group and model group, respectively. Two weeks later, all rats were killed to observe following index: epidermal growth factor, nitrogen mon- oxidam, blood vessel endothelium, gastric mucosa, ulcer index and pathology of gastric mucosa tissue.Result:(1)Observation of gastric mucosa: Through calculating ulcer index and ulcer inhibition ratio, we found that there was therapeutic effect on ranitidine group, lower dose group and large dose group, but it was the most manifestation in large dose group.(2)Observation of gastric mucosa tissue: HE tincted microtome section: Model group: ulcer in progression period, ablated mucosa layer, some ulcer reached to muscular layer, inflammatory necrosis, exfiltration, inflammatory cell infiltration, unobvious hyperaemia. Ranitidine group: ulcer in convalescent period, some regenerated glandular organ in borderline showed cryptomere distension and neogenesis vascellum was abundance in granulated tissue. Low dose group: ulcer in convalescent period, necrotic tissue was decreased in ulcer surface, regenerated glandular organ was irregularly distented, mucus secretion was abundance, granulation tissue hyperblastosised, inflammatory cell infiltration and abundanced in micrangium. Large dose group: ulcer in convalescent period, ulcer diminished, regenerated glandular organ in borderline showed cryptomere distension, quantity of regenerated glandular organ was increased, tela cellulose diminish- ed, quantity of collagenous fibrils was increased and abundance in micrangium.(3)Level of EGF,NO in low dose group, large dose group and ranitidine group was markedly increased than that of model group(P<0.05),while ET level was markedly dropped in those groups than that of in model group(P<0.05) .Conclusion: Jiedu Huoxue prescription can increase EGF in gastric mucosa tissue and in blood serum, induce and promote synthesis of NO and inhibit releasing of ET.All of these can restrain gastric acid secretion,adjust phlegm-hydrocarbonate barrie,promote gastrointestinal mucosa epithelial proliferation, maintain gastroin- testinal integrality, protect gastric mucosa, improve microcirculati- on of gastric mucosa, increase oxygen supply. So Jiedu Huoxue pr- escription can promote ulcer henosis and enhance quality of ulcer healing.
