| The first departmentObjective: To study the therapeutic effect of Huazhuo Jiedu Xiaokui Jian Prescription on Gastric Ulcer.Method:The case-control study was adopted.The selected cases according to"Randomized Number Scale"were randomly divided into two groups:50 patients in treatment group were administered with Huazhuo Jiedu Xiaokui Jian Prescription;50 patients in control group were administered with Ranitidine. Both groups were treated for four weeks.Then to observe the clinical symptoms'changes for carrying out gastroscopy to evaluate the therapeutic effects.Result: (1)Clinical symptom therapeutic effect:In treatment group the total efficiency is 86%,which is significantly higher than that of control group(P<0.05).In control group the total efficiency is 74%.(2)Compared the main clinical symptom before and after treatment:All the main clinical symptoms abscess and sour regurgitation were improved after treatment in control group. There was a significantly difference between treatment group and control group after treatment in dry mouth and constipation.(3)Gastroscopic therapeutic effect: In treatment group the total efficiency is 96%,which is significantly higher than that of control group(P<0.05).In control group the total efficiency is 74%. (4)Compared ulcer prostective efficacy: the ulcer recurrence rate in treatment group was 9.8%while in control group was 13.9%.There was a significantly difference between two groups. (5)Evaluate therapeutic safety in the two groups:There were neither obvious changes in blood, urine or stool rules before and after treatment or in Electrocardiogram or liver-rental function. In the course of the treatment, all patients had no undesirable response, which pointed the fact that Huazhuo Jiedu Xiaokui Jian Prescription has no toxic and side effect. Conclusion:The study indicates that Huazhuo Jiedu Xiaokui Jian Prescription can improve patient's clinical symptom, improve quality of ulcer healing to decrease the recurrence rate of Peptic Ulcer. Its curative effective aspect approves that Huazhuo Jiedu Xiaokui Jian Prescription is feasible and practicable in treating Gastric Ulcer.The Second DepartmentObjective:To explore mechanism of Huazhuo Jiedu Xiaokui Jian prescription in treating gastric ulcer by constructing animal model .The aim is to provide experimental evidence for clinical research.Method: 40 healthy male SD rats were selected and randomly divided into five groups, each group had 8 rats. Rats models of gastric ulcer were established by hawkinsin.24 hours after the operation, the amount of dried medicinal herb given to large dose group and low dose group was 22.1mg/(kg·d)weight and 5.5mg/(kg·d)weight respectively. The amount of ranitidine given to ranitidine group was 31.25mg/(kg·d).The distilled water was given to model group, the amount of distilled water was 10mg/(kg·d).Ten days later, all rats were killed to observe following index: nitrogen monxidum, blood vessel endothelium, gastric mucosa, ulcer index and pathology of gastric mucosa tissue.Result:(1)Observation of gastric mucosa: Through calculating ulcer index and ulcer inhibition ratio, we found that there was therapeutic effect on ranitidine group, lower dose group and large dose group, but it was the most expressly in large dose group.(2)observation of gastric mucosa tissue: HE staining microtome section: Model group: ulcer in progression period, ablated mucosa layer; some ulcer reached to muscular layer, inflammatory necrosis, exfiltration, inflammatory cell infiltration unobvious hyperaemia. Ranitidine group: ulcer in convalescent period, some regenerated glandular organ in borderline showed cryptomere distension and neogenesis vascellum was abundance in granulated tissue. Granulated tissue. Low dose Group: ulcer in convalescent period, necrotic tissue was decreased in ulcer surface, regenerated glandular organ was irregularly distent, mucus secretion was abundance, granulation tissue hyperbastosis, inflammatory cell infiltration and abundanced in micrangium. Large dose group: ulcer in convalescent period, ulcer diminished, regenerated glandularorgan in borderline showed cryptomere distension, quantity of regenerated glandular organ was increased, tela cellulose diminished, quantity of collagenous fibrils was increased and abundance in micrangium. (3)Level of NO in low dose group, large dose group and ranitidine group was markedly increased than that of model group (P<0.05),while ET level was markedly dropped in those groups than that of in model group(P<0.05).Conclusion: Huazhuo Jiedu Xiaokui Jian prescription can induce and promote synthesis of NO and inhibit releasing of ET. All of these can restrain gastric acid secretion, adjust phlegm-hydrocarbonate barrier, promote gastrointestinal mucosa epithelial proliferation, maintain gastrointestinal integrality, protect gastric mucosa, improve microcirculation of gastric mucosa, increase Oxygen supply. So Huazhuo Jiedu Xiaokui Jian prescription can promote ulcer henosis and enhance quality of ulcer healing.
