The urban community health service (CHS) is a grass roots organization that takes the people's health as its center and takes people's health need as its guide. To develop the urban CHS can increase the equity and efficiency of health service, solve the problem of structural imbalance in urban health service system, and enhance urban sanitary service system. Additionally, it can control the unreasonable growth of the medical expenditures and raise the residents'health level as well. To develop the urban CHS is also the urgent need for building up the residents'medical insurance system in towns and cities, and the fundamental path to adapting changes of disease family and achieving the goal of everyone to possess the equal right for health care. On February 8th 2006, the State Council studied the development CHS in executive meeting. And the meeting ,'the guidance on the development of urban community health services" was discussed and approved in principle. This guidance has raised new requirements on the development of urban CHS. For that purpose, the author has made research on current situation and developmental strategies of urban CHS.Having developed its preliminary framework, the urban CHS has been experiencing a transition from the initial stage to maturation stage. Having had a certain scale, CHS has been basically equipped with CHS management information system, provided residents with basic health services and public health services. The main channels of financing of CHS are government financial subsidies, fee-for-CHS, street committee's support, medical insurance funds. In addition, the government has promulgated many policies that promote CHS to develop. The basic medical insurance system for urban workers and CHS supplement and accelerate each other. The urban CHS is gradually improved.However there exist some difficulties and problems that seriously hold back the development of the urban CHS:First, the CHS management system has such deficiencies, as overlapping of management institution, inadequate supervision and lack of professional CHS managers.Second, in large cities, some community health services are not in place, and their utilization rate is low. In the moderate cities and small cities services provided by CHS are mainly the diagnosis and treatment of common diseases and frequent diseases. Nevertheless the other basic CHS items are very little or no. |
