Objective: The focal point of health reform is payed close attention to extensively by society currently. The key to health contradiction is the difficulty in seeing doctors and the high cost of health. The increasing speed of health is 10% per year. The competition of medical market is more and more impetuous pregressively. Faced the current situation, the clinical pathway may be the relatively effective method in the process of monopathy diagnosis and treatment. This research has systematically investigated the flow design, organization and practice, process control, effects of application of hemorrhoid's clinical pathway, has explored the way for the practice of hemorrhoid's CP corresponding to the actual situation in China, has exerted the practical significance in enhancing the medical quality, specifying the process of diagnosis and treatment, shortening the hospital days, lowering the medical cost, advancing the reasonably utilization of medical resources, and has provided experience for making and practicing CP of other diseases.Method:This research project is based on the experience of studying and application of CP from hospitals home and abroad, and selects hemorrhoid from the largest hospital experting in archos and enteropathy diseases in Dalian. First of all, with the aid of hospital information system, we retrieved the information on hemorrhoidal patients from January 1st to June 30th, 2006. Then, according to the theories and methods such as Median, Evidence-based Medicine, Vilfredo Pareto Principle, Delphi Method and so on, we designed the hospitalization time, contents and items in diagnosis and treatment flow of CP, which were put into practice in clinicafter adequate propaganda and training as well as consummating the system of organization in August, 2006. After a period of practice (5 months), we systematically analyze and evaluate the data by the hospital information system, including hospitalization time, preoperative waiting time, days of injecting antibiotics intravenously, total hospitalization expenses, expense of each item, degree of patients'satisfaction, convalescence rate, confirmed diagnosis rate within 3 days, diagnostic coincidence rate of discharging and adimitting, the first degree medical record rate and so on.Result: There are 56 patients in CP group and 55 patients as control group drawn from 213 patients in traditional group which amount to be 111 patients in total. There are no significant differences between the two groups in terms of sex and age. 1. The hospitalization time, preoperative waiting time, days of injecting antibiotics intravenously in CP group are shorter than that in traditional group. There are significant differences between the two groups. 2. Total hospitalization expenses and each item expense of hospitalization are less than that in traditional group. There are significant differences between the two groups. 3. There are no significant differences between the two groups on the degree of patients'satisfaction. 4. There are no significant differences between the two groups on convalescence rate, confirmed diagnosis rate within 3 days, diagnostic coincidence rate of discharging and adimitting, the first degree medical record rate.Conclusion: The clinical pathway, an advanced model of medical quality management, which does not weaken the clinical care effect, may definitely shorten everage hospitalization period, lower the average cost of medical treatment, promote reasonable utilization of medical resources and coincide with the policies of medical health reform of"low cost and wide coverage". Meanwhile, it has also actual sense for clinical pathway on advancing normalization of diagnosis and treatment, promoting the co-operations among medical staff and the communications between doctors and patients, helping the hospital management to obtain continuous improvement and coping with the policy on price limitation of monopathy issued by medical insurance department.
