Objective:To evaluate the safety and effectiveness of automatic functions of Kappa pacemaker and to their clinic value.Methods:The study enrolls thirty-eight patients who were implanted DDD(R) Kappa pacemaker for sick sinus syndrome , atrio-ventricular conduction block, sick sinus syndrome with atrio-ventricular conduction block from January 2002 to June 2005, male 23, female 15, age 40-85 years, mean 66.1 ±11.5 years. The automatic functions in Kappa700 pacemaker are not same in Kappa400 pacemaker. The patients were divided into Kappa700 group and Kappa400 group consecutinely. Observe AV delay, ventricular threshold, ventricular output voltage, atrial sensitivity, ventricular sensitivity, mode switch number, the heart function, changes of echocardiography, scores of QOL from baseline, one week, one month, three months, six months, one year by self-paired design and design between groups.Results: At the end of the follow-up, Echocardiography changes: Kappa700 group have changes obviously in LV, LVESD, LVEDD, EF(P<0.05). Kappa400 group have no change obviously in LV, LVESD, LVEDD, EF(P>0.05). Compared with Kappa700 group, AV delay was 159.9±46.5ms after one year. Kappa400 group was 161.5 ±13.7ms, no obvious change. Their standard deviations(SD) was not same. Compared with Kappa700 group, the mean of output puse width was 0.33-0.35ms(Kappa400 group 0.4-0.6ms) , the mean of output voltage 1.25-1.33v(Kappa400 group 2.5-5v) , the mean of atrial sensitivity 0.73-0.78mv(Kappa400 group 0.5mv) , the mean of ventricular sensitivity 3.84-3.95mv(Kappa400 group 2.8mv). The pacing and sening functions of them were normal. kappa700 group had 5 patients who happened to mode switch., 534switchs. Kappa400 group had 4 patients who happened to mode switch, 511switchs.Kappa400 group have no mode switch for small swings of F wave and proxysmal atrial flutter. Adjusting parameters of pacemaker, start up the mode switch successfully.
