Objective: To evaluate the effect of different pacing mode(VVI and DDD)impact on the prognosis of elderly patients with pacemaker implantation.Methods: 60 permanent pacemaker implanted elderly patients who come form Maanshan People's Hospital with cardiac conduction system disease and divided into dual-chamber pacemaker group(27 cases)and Single-chamber pacemaker group(33cases).Echocardiography parameters(LAEDD,LVEDD,LVESD,LVEF),NT-pro BNP and SF-36 were used as the observation parameter,according to the preoperative and postoperative half a year,1 year abovementioned program to evaluate the cardiac function and the quality of life,analyse the changes and correlations before and after the challenge of pacemaker implantation between the two groups.Results:1 The postoperative changes appeared on the heart diameter of the patient's heart(LAEDD,LVEDD,LVESD)between single-chamber pacemaker group and dual-chamber pacemaker group1.1 Postoperative half a year[LAEDD(40.52±7.57 VS 39.59±4.80,P>0.05),LVEDD(50.9±5.53 VS 52.30±5.68,P > 0.05),LVESD(38.06±3.70 VS39.44±6.76,P>0.05)]1.2 Postoperative 1 year [ LAEDD(42.00±6.77 VS 41.15±6.05,P > 0.05),LVEDD(50.39±5.77 VS 53.74±8.66,P > 0.05),LVESD(38.64±5.52 VS41.41±9.22,P>0.05)]2 The postoperative changes appeared on the cardiac function(LVEF ?NTpro-BNP)between single-chamber pacemaker group and dual-chamber pacemaker group2.1 Postoperative half a year[LVEF(53.73±5.33 VS 51.81±9.38,P>0.05),NTpro-BNP(235.73±112.20 VS 228.63±107.74,P>0.05)]2.2 Postoperative 1 year[LVEF(53.45±6.74 VS 50.37±9.37,P>0.05),NTproBNP(257.82±165.28 VS 248.33±136.74,P>0.05)]3 The postoperative changes appeared on the quality of life(General Health)between single-chamber pacemaker group and dual-chamber pacemaker group3.1 postoperative half a year[General Health(78.33±9.63 VS 79.22±10.29,P>0.05)]3.2 postoperative half 1 year[General Health(85.97±6.92 VS 86.15±7.75,P>0.05)]Conclusion:1 Single-chamber pacemaker and dual-chamber pacemaker for elderly patients with no significant change in cardiac diameters.2 Single cardiac pacemaker and dual-chamber pacemaker implantation in elderly patients with cardiac ejection fraction decreased,NTpro-BNP increased were not obvious,no significant reduction in cardiac systolic function.3 Single-chamber pacemaker and dual-chamber pacemaker significantly improved the symptoms and the quality of life of elderly patients,but there were no difference between the two group.
