[Backgroud]Severe acute pancreatitis(SAP) often leads to multi-organ dysfunction syndrome(MODS). Among important non-pancreas organs' function disorder, renal dysfunction (nephropathia pancreatica) accounted for 14%-43%, only less than lung dysfunction. If acute renal failure(ARF) took place, the mortality could arrive 71%-84%. At present, the nosogenesis isn't clear completely. Some research results showed that the factors involved in SAP renal injury included the renal hemodynamic abnormality, mediators of inflammation , cytokine, TNFα, PLA2, PAF, ET, NO, PGI2, TXA2 and apoptosis. Apoptosis and its related gene are nowadays hot points in biomedicine field and gained more and more attention. Animals experiments showed that, among diabetes rats, heart failure rats, adriamycin nephronia rats and endotoxin-poisoned rats, renal apoptosis increased, expression of apoptosis-promoted gene increased, while expression of apoptosis-restrained gene decreased. The number of apoptosis cells often formed direct ratio with the severity of renal function injury. This indicates that, in the secondary renal injury, decrease of renal cell quantities and disorder of renal function are probably caused by apoptosis.
