Objective:1.To explore the change in apoptosis of pancreas and kidney changes after application of glycyrrhizin in rats with severe acute pancreatitis;2.To evaluate the therapeutic effect of glycyrrhizin on pancreatitis.Methods:72 adult male SD rats were divided into sham operation group (sham operation group, SO), control group (severe acute pancreatitis, SAP), the treatment group (glycyrrhizin-treated group, GL) randomly (random table method). SAP group were injected 5% sodium taurocholate 1ml/kg into pancreatic duct retrogradly for establishing severe acute pancreatitis model, GL group were injected glycyrrhizin (2mg/ml, 100mg/kg)immediately in the tail vein after molding, SAP group were injected isodose physiology saline immediately in the tail vein after molding. Peritoneal cavity of So group were closed after laparotomy flip duodenum, spile in pancreatic duct, no injection of sodium taurocholate, and the remaining steps same with model group, Kill and dissect the rats at 6,12,18 h time points, observe the change of the organs, obtain the pathological changes of pancreas and kidney by HE staining, measure the serum amylase (amylase AMY) and creatinine (creatinine Cr) by fully automated analyzer, ELISA detect interleukin-6 (interleukin-6, IL-6),detect the apoptosis of pancreatic acinar and renal cell by TUNEL method. Establish a database using statistical software SPSS16.0, compare mean data by using completely randomized design analysis of variance, Comparison between groups with DunnettT3 method, Apoptosis and renal damage associated with the correlation analysis, P <0.05 as significant difference.Result: 1.In SO group, all rats surviving before being killed, had no pathological damage on pancreas and kidney, normal serum amylase and Cr, low expression of IL-6 ,a few apoptosis occasionally.2. In SAP group, two rats died in 18h period of time after SAP model established, pathological changes of pancreas and kidney were significantly, higher expression of IL-6 values ,serum Amy and serum Cr were higher than that in SO, and were significantly high compared with SO group in the 18h point. The apoptotic index at each time point were significantly higher than that in SO group (p <0.01). Pancreatic acinar cell apoptosis decreased with the proceeding of the course. The apoptotic index were negatively correlated with pathology score, serum amylase and IL-6 concentrations; but renal cell apoptosis index increased gradually with the prolongation of the course. The apoptotic index were positively correlated with the pathology score, Cr and IL-6 concentrations.3. In GL group, all rats survived before executed. The survival rate was 100%. The pathological changes of pancreas and kidney were slighter than that in SAP group, the Cr,IL-6 and amylase, compared with SAP group were significantly different (p <0.05). The apoptotic index of pancreatic acinar cell at each time point were higher than SAP group (differences were p <0.01; p <0.01; p >0.05), present a gradual downward trend with the proceeding of the course. The apoptotic index of Kidney cells at different time points were lower than that in SAP (p <0.01), has increased gradually with the proceeding of the course.Conclusion:1. GL may induce pancreatic acinar cells apoptosis in rat with SAP, and inhibit apoptosis of kidney cells contemporary.2. GL may reduce inflammation and improve the organ function in rats with SAP. The mechanism of glycyrrhizin treatment for pancreatitis might be achieved by bring into play hormone-like effects, stabling of lysosomal enzymes, enhancing capacity of cells against damage, reducing the release of inflammatory factors in early SAP, reducing inflammation, raising the trigger points of SAP waterfall cascade of inflammatory mediators, so as to reducing inflammation of the organs damage.
