Clinical Study Of JianPiLiDan Decoction In Treating Chronic Viral Cholestatic Hepatitis

Objective :To analyse the changes of the level of serun total bilirubin (STB), conjudated bilirubin (CB), alanine aminotransferase(ALT), total bile acid (TBA) and alkaline phosphatase (ALP) of patients with Chronic viral Cholestatic Hepatitis (CCH) after treatment with JianPiLiDan decoction (JPLD) and observe the changes of clinical symptoms, then explore its remedical effect and mechanism to Chronic viral Cholestatic Hepatitis. Method :63 cases came from the Department of Hepatitis of The Afiliated People's Hospital of YUNYANG College of Medicine Technology from March 2004 to November 2005.A11 cases were diagnosed Chronic viral Cholestatic Hepatitis according to laboratory examination and clinical symptoms. Patients with Chronic viral Cholestatic Hepatitis were divided into two groups in light of the order of seeing doctor in random. One was control group(32 cases).the other was observe group(31 cases) .All patients were requested to rest in bed as long as possible,take digestive food ,no drinking, not taking any medicine which can do harm to liver definitely. All patients were given TabVitamin B Co 2^# tid and Tab Glucurone 0.1g tid . patients whose STB exceed 171. 2 umol/L were given Inj VitaminK₁ 10mg im qd, whose HBV-DNA exceed 500COPY/L were given Tab Lamivudine, 0.1g,qd, and when there were virus variation, patients would be given Tab Adefovir dipivoxil 10mg, qd . The control group were served by JPLD decoction(Astragalus membranaceus 25g,codonopsis pilosula 25g,Atractylodes macrocephala 15g,Lysimachia christinae 20g,Artemisia capillaries 15g,Gardenia jasminoides 10g,Rheum palmatum 10g,Bupleurum chinensis 5g,Paeonia lactiflora 20g,Ligustrum lucidum 15g,Poria cocos 10g,Lygodium japonicum 15g,Curcuma phaeocaulis 12g,Paeonia lactiflora 25g,Trichosanthes kirilowii 15g,Whitmania pigra 6g,Tabanus bivittatns 6g, Glycyrrhiza uralensis 10g. Abdominal distension plus Magnolia officinalis 10g,Raphanus sativus 15g; itch plus Polygonum aviculare 15g,Pophora flavescens 15g,Saposhnikovia divaricata 10g; Hepatalgia plus Melia toosendan 10g,Corydalis yanhusuo 15g;Anorexia plus Crataegus pinnatifida 20g,Hordeum vulgare 15g,Oryza sativa 20g,Amomum villosum5g) one dose everyday, which was heated in higher temperature for about ten minutes firstly ,then heated in lower temperature for about twenty five minutes , drinking 150ml in the morning and 150ml at night separately. The observe group were treated with Tab Ursodeoxycholic Acid (UDCA) 150mg tid and Inj Diammonium Glycyrrizinate 30ml in 250ml 5% glucose injection iv drop qd. The course of treatment of two group was 40 days .Compared the levels of STB, CB, ALT, TBA, ALPand curative effect between two groups in twenty days and forty days separately. Results:(1)The cure rate in the control group was 28.13%,and its total effective rate was84.38 %,significantly higher than that of the observe group, cure rate9.68%and total effective rate 51.61% ( P<0.05 ) .(2) Analysis of the clinical symptoms of Chronic viral Cholestatic Hepatitis patients demonstrated that the symptoms such as fatigue , anorexia etc were different obviously compared with that of the healthy persons ( P<0.05 ) , clinical symptoms have been meliorated dramatically in the control group after treatment. we could find significant differences as Compared all the clinical symptoms scores between two groups .(3) The levels of STB, CB, ALT, TBA, ALP were markedly increased in the Chronic...
