| Objective: To observe the effect of lingyuhua decoction on chronic viral hepatiti s b(CHB)patients with anxiety disorder and explore its mechanism.Methods: The patients with CHB and anxiety disorder who met the inclusion cri teria from November 2016 to March 2019 were randomly divided into the treat ment group of lingyuhua prescription and the control group of xiaoyao pill.The treatment group was treated with lingyuhua prescription,and the control group was treated with xiaoyao pill for 12 weeks.The two groups were treated with 2courses of treatment,a total of 24 weeks.The changes of anxiety state psychol ogical scale(HAMA,SAS)score and TCM syndrome score before and after trea tment were observed,and the changes of serum hbv-dna,HBs Ag quantification,HBe Ag quantification,ALT,AST,ACTH,Cor and eye movement examination b efore and after treatment were detected respectively for efficacy evaluation.Results:(1)Both HAMA score and SAS score of anxiety status of patients in t he two groups were improved compared with that before treatment(P<0.05),the total effective rate of HAMA in the treatment group was 88.10%,the total effe ctive rate of SAS was 90.48%,the total effective rate of HAMA in the control group was 67.5%,and the total effective rate of SAS was 70%,the treatment gr oup was better than the control group(P<0.001).The improvement of HAMA a nd SAS scores was proportional to time,and the decrease trend in the treatment group was significantly better than that in the control group(P<0.001).(2)after treatment,hbv-dna quantification in both groups decreased compared with that b efore treatment(P<0.05),the treatment group was superior to the control group(P<0.05),and the proportion of hbv-dna logarithmic value decreased more than 2 log S/CO in the treatment group was higher than that in the control group(P<0.05).After treatment,the quantitative HBs Ag in both groups decreased compared with that before treatment(P<0.05),and the logarithm value of HBs Ag in the tr eatment group decreased by more than 0.5log S/CO and 1 log S/CO than that i n the control group(P<0.05).After treatment,there was no significant decrease i n HBe Ag in the two groups(P >0.05),but the proportion of HBe Ag in the trea tment group decreased by more than 0.5log S/CO was higher than that in the co ntrol group(P<0.05).(3)the total effective rate of ALT was 78.6% in the treat ment group and 55% in the control group,both of which were lower than befor e treatment(P<0.05),and the treatment group was better than the control group(P<0.05).AST decreased in both groups compared with that before treatment(P<0.05),and the treatment group was superior to the control group(P<0.05).(4).Compared with the pre-treatment group,ACTH and Cor decreased significantly(P<0.05),and the treatment group was superior to the control group(P<0.05).(5)the TCM symptom scores of both groups were significantly improved compared with those before treatment(P<0.001),and the treatment group was superior to the control group(P<0.001).6)the abnormal rate of patients in the eye moveme nt test is 75%.Conclusion: Lingyuhua decoction can significantly improve and alleviate the anxi ety state of CHB patients with anxiety disorder,promote the recovery of hepatiti s b,inhibit hbv-dna,reduce HBs Ag and HBe Ag quantification,improve and pro mote the recovery of ALT,AST,reduce ACTH and Cor;"Heart,liver and kidne y three zang tongzhi" lingyuhua prescription is an effective prescription for CHB with anxiety disorder.Its mechanism of action may be related to antiviral and r egulating neuroendocrine functions. |
