| Objectives:1. To investigate the safety and effectiveness of TecnisZ9001 in treatment of phacoemulsification for the patient with senile cataract.2. To investigate the difference of postoperative aberration and visual function between two kinds of IOL: TecnisZ9001and SN60ATIOL.Methods:1. Fifty senile cataract patients(50 eyes) at the age of 50 to 80 years old were selected and received preoperative examination of cataract surgery. Meantime, they were classified randomly into two groups and treated with phacoemulsification. One group was implanted an intraocular lens with a modified anterior surface(TecnisZ9001, AMO) and other group was implanted an IOL with biconvex spherical surfaces (ACRYsofSN60AT, ALCON). All patients accepted phacoemulsification combined with implantation of IOL by one doctor.2. After 2 months, 50 patients accepted the wavefront aberration measurement by OPD-Scan-ARK-10000 device .The following indexes: best corrected visual acuity,pupil size,high and low contrast visual acuity. Contrast visual acuity include three rates(20%, 40%, 60%).3. The indexes were analyzed by spssl3.0 Statistics software,and the indexes of two groups were compared with T test or x2 test.Results :1. 50 patients were examined at all follow-up visits. No complication was found operatively and postoperatively. No posterior capsule opacification (PCO) > IOL decentration and apparent tilt were founded.2. Mean age of two groups were 68.16 ± 8.24(Y) and 67.64 ± 7.54(Y) respectively. There were no significant difference between two groups (p=0.82).Mean dipoter of IOL were 20.26+1.71D and 20.18±2.16D, the difference between two groups were not significant(p=0.89).3. Mean best corrected visual acuity(BCVA) of Tecnis group after 2 month was 0.91±0.14,which of SN60AT group was 0.80 + 0.12. BCVA of both groups were significantly improved, but Tecnis group got better BCVA than SN60AT group(p=0.02). Mean pupil size of two groups when accepting different Contrast visual acuity examination were 4.30+1.30 and 4.22 + 1.30 respectively,two groups had no significant difference(p=0.77). Mean BCVA under contrast 20%> 40%> 60% of Tecnis group were 0.55 + 0.11, 0.64 + 0.11, 0.80+0.1, SN60AT group were 0.25 + 0.08, 0.36 +0.09, 0.53 + 0.15) respectively. The BCVA under contrast 20%, 40%, 60% of Tecnis group were better than those of SN60AT group and there were significant differences (pO.Ol).4. Mean aberration of Tecnis group was smaller than that of SN60AT group when pupil size is 3.0mm ,but there was no significant differences (p>0.05).Mean aspherical aberration of two group when pupil size was5.5mm were O.OOlumi 0.045um, 0.101um±0.138um respectively;Mean vertical coma of two group were 0.03um ± 0.06um, 0.15um±0.19um respectively, Mean horizontal coma were 0.02um ± 0.06um and 0.14um± 0.23um. The differences were significant by T test analysis, (p=0.01, 0.04, 0.028).Conclusion:1. It is one of effective ways to combine phacoemulsification with TecnisZ9001 aspherical intraocular lenses implantation for treatment of senile cataract.2. The clinical results indicate that the spherical aberration of the eyes after cataract surgery can be eliminated by modifying the anterior surface of the IOL. The TecnisZ9001 lens compensates for the positive spherical aberration in elder eyes and improves different contrast visual acuity, thus gets better visual performance.
