Objective:To measure the amount of tilt and decentration of aspheric intraocular lens (IOL) in the lens capsule using the Pentacam Scheimpflug System after cataract surgery and effect on higher-order aberrations (HOA) and visual qualityMethods:1,A total of 120 eyes (90 patients) were divided into 4 groups randomly, of 30 eyes in each group, one group with aspheric IOL Tecnis ZA9003,one group with aspheric IOL Acrysoft IQ,one group with aspheric IOL Akreos AO, one group with spheric IOL AR40e. The amount of tilt and decentration of IOL was measured using the Pentacam Scheimpflug System 1 week,1 and 3 months postoperatively.2,A total of 60 eyes(50 patients)were divided into 2 groups randomly, of 30 eyes in each group, one group with Sensar AR40e (AMO), one group with Tecnis ZA9003 (AMO).3 to 4 months after surgery, higher-order aberrations (HOAs) and modulation transfer function(MTF) at six spatial frequencies (5,10,15,20,25, 30cpd) were measured in 5.0mm pupil size.Tilt and decentration of the IOLs were measured using Scheimpflug photography. The tilt and decentration, HOA and subtracted lower order aberration MTF(MTF,HOA))were compared. The effect of tilt and decentration on HOA and visual quality was assessed using multiple regression analysis.Result:1,There were four groups:ZA9003 group, IQ group, AO group and AR40e group. No significant differences in IOL tilt were found in each group after 1 week,1 month and 3 months follow-up time (P>0.05). Four groups at different follow-up time of the tilt, differences were not statistically significant (P> 0.05); No significant differences in IOL decentration were found in each group after 1 week,1 month and 3 months follow-up time (P>0.05).Four groups at different follow-up time of decentration, had shown AO group smaller than the ZA9003,IQ,AR40e and the differences were statistically significant (F=6.391,4.265,7.692; P=0.021,0.030, 0.011);ZA9003 group, IQ group and AR40e group had no significant difference between any two (P> 0.05);2,The mean optic tilt was (2.99±0.94)°for AR40e group and (2.98±1.15)°for the ZA9003 group. The mean optic decentration was (0.3±0.13)mm, (0.31±0.14)mm, respectively. No significant differences in IOL tilt or decentration were found (t=-0.044,-0.525,p=0.965,0.600); The total 4th order spherical aberration in AR40e group was larger than it was in ZA9003 group and the differences were statistically significant (t=-10.386,P=0.000); In 5.0mm pupil size, MTF(HOA) at all spatial frequencies in ZA9003 group were larger than they were in AR40e group and the differences were statistically significant (P<0.05); Tilt and decentration did not significantly affect HOAs and MTF(HOA) with either IOL.Conclusion:1,Pentacam Scheimpflug System can be used to dynamically observe and evaluate the position of IOL in the eye; The amount of tilt and decentration in eyes with a 1-piece acrylic aspheric IOL IQ with flexible haptics implanted in the lens capsule was similar to those in eyes with a 3-piece acrylic aspheric IOL ZA9003 with rigid PMMA haptics; 4-point haptic aspheric IOL AO maybe less than 2-point haptic aspheric IOL ZA9003,IQ;2,Under normal conditions the amount of tilt and decentration of spheric IOL AR40e and aspheric IOL ZA9003 in the lens capsule was small; When IOL tilt and decentration are within normal limits, they do not comprom-ise the correction of spherical aberration by the aspheric IOL and the amounts of tilt and decentration of both IOLs are not large enough to cause deterioration of visual quality.
