Purpose: To investigate the clinical features and evaluate the efficacy of surgical treatments of double depressor palsy.Methods: The clinical data, surgical techniques, pre-and post-operative clinical results were analyzed in 24 patients with double depressor palsy (DDP) collected between Mar.2002 to Apr.2006. The directed antagonist and/or yoke muscles of the paretic eye were weakened, or the paralysis muscles were strengthened. Clinical characteristics and surgical techniques used and the results were summarized.Results: After 3 to 12 months follow-up, strabismus were completed cured in 83.3% (20/24) of patients in our study and rest of 4 patients had successful results after secondary surgery. 17 of 18 patients with horizontal strabismus aligned nicely and one with V-sign was also resolved after operation. Two cases with DVD (2/8) were not fully corrected, but didn't affect the appearance. The compensatory head posture were disappeared after operation. 3 of 24 patients with pseudo-blepharoptosis in the primary positions were all improved.Conclusion: The major clinical feature of DDP is hyper-deviation and the non-paretic eye is often the fixation eye.
