Objective To explore the clinical meaning and serum changing of tumor necrosis factor- a in chronic congestive heart failure(CHF). Method 46 cases of congestive heart failure, of which 28 cases are male and 18 cases female, mean age ranging from 52~88 (62 ± 10.13), of which, 20 cases are coronary heart disease and 18 cases are rheumatic 4 cases are hypertension patients, and 3 cases are calcified valvular disease , 1 case is dilative myocardiopathy, sex and age matched health subject were chosen as control group; Cardiac function were classified by NYHA standard, 22 cases with cardiac function II, 13 cases with cardiac function III, and 11 cases with cardiac function IV,all hospitalized patients have no severe infection, and routine X-ray, film, EKG, Echo, and biochemical lab exam were done during hospitalized, and rule out the diagnosis of tumors, diabetes, liver and kidney diseases, connective tissue disease and hyperthyroidism and other complications, all patients received diauretics, digitoxin, ACEI, vasodilator agent, and |3 -blockers, control group were consist of 10 normal healthy people of male and female respectively, during health check up, age ranging from 49-75, mean age is (63.11 ± 6.11), Level of tumor necrosis factor was detected by using Enzyme linked method, and left ventricular ejection fraction (LVEF) were measured by two dimensional Echo. Result 1. serum level of tumor necrosis factor- a in study groups all increased, but no markedly difference in cardiac function II group with the control group(P>0.05), and there is a markedly difference in cardiac function III,IV compared with the control group (P<0.05), there is a negative relation ship with level of tumor necrosis factor- a in serum and LVEF(r=0.54, P<0.05).2. the worse cardiac function ,the high level of tumor necrosis factor- a is.
