Objective To evaluate the diagnostic value of ultrasound parameters of ovarian stromaarea(S), total ovarian area(A) and S/A ratio for polycystic ovaian syndrome(PCOS) And to study the endocrinologic and metabolic difference between insulin resistence and non- insulin resistence in women with PCOSMethods A prospective study involved total 30 PCOS patients as group A and non-PCOSinfertility patients as control (groupB) The A, S. and S/A ratio were determined by use of transvaginal pelvic ultrasound ,and serum levels of follicle stimulating hormone (FSH), luteinizing hormone(LH), testosterone (T) ,sex hormone binding globulin(SHBG), insulin(FI) . insulin-like Growth Factor- I (IGF- I ) and insulin-like Growth Factor Binding Protein- I (IGFBP- I ) were assayed in 30 PCOS patients and 30 controls Of 30 PCOS patients underwent glucose tolerance test,insulin-release-test, C-peptide-release-test. The above intentioned measurement was performed again in those women who showed insulin resistence(IR) after 12 weeks of metformin therapy .and the other patients(non-insulin resistence) were treated with CPAResult Patient with PCOS showed significantly higher ovarian S and S/A ratio whencompared to the control group(P<0.05, respectively) S/A ratio and S were significantly related to OGTT results and T. Serum fasting insulin concentrion,IR,and BMI.WHR in insulin resistence group were higher than non- insulin resistence group; LH ,SHBG was lower than non- insulin resistence group( p<0.05). Serum levels of FI,IR,AUC insulin, T and ovarian S and S/A ratio were decreased in insulin resistence group by metformin therapy. Serum levels of LH. T and ovarian S and S/A ratio were decreased and SHBG was increased in non-insulin resistence group after 12 weeks CPA therapy (p<0.05)Conclusion The S/A ratio and S are of value in diagnosis of PCOS. The insulinresistence group showed high levels of BMI,WHR, insulin and insulin resistence than that in non-insulin resistence group.The non- insulin resistence group showed mainly high levels of LH. Serum insulin concentration can be decreased and the sensitivity of insulin can be increased arfter metformin therapy. Serum LH and T concentration in non-insulin resistence group can be decreased by CPA therapy...
