Primary bile duct stone,a notion relative to secondary common bile duct stone that come from gallbladder . In this scientific dissertation, Primary bile duct stones include hepatolithiasis , primary choledocholithiasis and choledocholithiasis which come from hepatolithiasis. This disease pertains to the categories of costa-ache in Traditional Chinese Medcine.Primary bile duct stone is prevalent in clinic, Complicated and difficult to cure.It is the residual and recurrent stones which make the treatment become diffcult and make the disease with a gloomy prognosis. The mechnism of Primary bile duct stone is not clear, include inflammation, metabolism, immunity and mutation of gene pathological process ,relate to malformation of bile duct, infection of germ and parasite, silt up of the bile et al.Current treatment to Primary bile duct stone is surgical operation .But the single opreation couldn' t fulfil the require to change the quality of bile and prevent recurrent stones . Traditional Chinese Medcine has a great advantage in this aspect.It could ameliorate the metabolism of liver and change bile component, prevent the bile to form hepatolithiasis. So there is an important society significence through develeping the combination treatment with traditional Chinese medicine and operation to treat Primary bile duct stone.ObjectiveTo study the effect of SI-NI-SAN on bile component of postoperational patients of, probe into the mechanism of SI-NI-SAN in prevent of the form of primary bile duct stones, and develop the clinical adhibition of traditional medicine. MethodsTotal 33 patients who had suffered from primary bile duct stone undergo an opened abdominal operation to take away the stone, with a T tube installed in the common bile duct. All the patients were divided into two groups at random, the group 1 (administer traditional medicine SI-NI-SAN) and the group 2(blank group). The group 1 administer medicine in the fifth day after operation. Reserved bile of all the patients in the fifth, sixth, eighth and tenth day respectively after operation.A quantitative analysis was given after all the sample reservation were completed, to assay the concentration of total biliary acid, bilirubin, cholesterol, calcium, IgA and IgG in the sample. ResultsNo side-effect after administering the traditional medicine SI-NI-SAN. Significant change of the colour and quality of the bile that drainage from the T tube were not found. Before administering SI-NI-SAN, there was no significant difference between the two groups (p>0.05); After administering SI-NI-SAN, in the first day, there was no significant difference between the two groups(p>0.05); On the third day, there has been significant difference between the two groups in total biliary acid (p<0.05); Onthe fifth day ,there has been significant difference between the two groups in totalbiliary acid (p<0.01),indirect bilirubin (p<0.05)and IgG(p<0.05).ConclusionSI-NI-SAN can do effect of the liver and change the component of the bile to prevent the form of pigment stone.
