Objective To survey the biliary pathogen and their sensitivity to antibioties in patients with primary bile duct stone and to understand the positive rate of bacterial culture and the influence factors in patients with primary bile duct stone.Methods Microbes automatic detection system was applied to detect the spectrum of bile pathogen and their drug sensitivities in 180 cholelithiasis patients from January 2005 to December 2008.Results 148 specimen common cultures were positive in 180 specimen (82.2%),including 127 gram-negatives trains(85.8%),20 gram-positives trains (13.5%) and 1 fungus(0.7%). The most common pathogenic bacteria were Escherichia coli(57.4%) and Klebsiella pneumoniae(14.9%). The result of drug sensitivity showed the resistant rates of gram-negative strains to Piperacillin/taz,Cefoperazone/sul,amikacin,meropenem and imipenem is less than 5%. In this study gram-negative strains were highly resistants to the piperacillin,ciprofloxacin,levofloxaein and bactrim(>70%). The resistant rates of gram-positive to amphemycin, penieillin,ampicillin, vancomycin and teicoplanin is low. And the resistant rates of ciprofloxacin, levofloxacin and erythromycin is high.Conclusion There was an increasing percentage of positive cultures in the patients with course of disease≥10 years,having history of biliary tract surgery, bile duct inflammation and received antibiotic treatment. The main pathogen in patients with primary bile duct stonewas gram-negative,the drug resistant rates is rising.
