Objectives: Post-stroke depression is a common complication of stroke. Occurrence of post-stroke depression is related to many factors, there are a lot of diagnostic criterions and diagnostic scales in diagnosing post-stroke depression. MMADD(Multimodel approach to diagnosis of PSD) is a new scale compiled for diagnosis of post-stroke depression recently. We will investigate the incidence and the related factors of post-stroke depression by using MMADD and research the reliability and the validity of MMADD.Methods: 100 stroke inpatients who corresponded with entrance standards were admitted. Above all, age, sex, occupation,culture lever, economic status, family relation, medical insurance,course of stroke,muscle force of superior and lower limb and motor function of superior and lower limb were investigated by the first neurologist, and the activities of daily living (ADL) was assessed with Barthel index, cognitive impairment was measured with Mini-Mental State Examination, neurologic impairment was assessed with the NIH stroke scale(NIHSS). features of lesion(type of stroke,lesion volume, side of lesion, lesion stratum, number of lesion, the minimum distance of the anterio border of the lesion to the frontal pole and the ratio of the distance of the anterio border of the lesion to the frontal pole and the distance from the frontal to occipital pole) were measured according to computer tomographic (CT) scan of the brain. These stroke patients were assessed with MMADD by two neurologist at the same time, then the depression of these patients also were assessed with DSI,HAMD,CCMD-3,DSM-â…£ by the first neurologist, these patients were repeatedly assessed with MMADD by the first neurologist after three days. Analysis of single factor and multiple stepwise Logistic regression were undertaked to screening the related risk factors associated with depression. For reliability of MMADD, test-retest reliability, Cronbach's alpha coefficient,objectivity reliability(inter-rater reliability)and precision reliability(intra-rater reliability)were studied. For validity of MMADD, content validity, criteria validity(convergent validity)and construct validity were studied.Results:The prevalence of the post-stroke depression was 62% in this research. Analysis of single factor indicated that the occurrenc of PSD were related to sex(P=0.048), ADL (P =0.0001), cognitive impairment(P=0.0464), neurologic impairment (P=0.0007), muscle force of superior limb (P=0.014), muscle force of lower limb(P=0.006), motor function of superior limb(P=0.047), motor function of lower limb(P=0.004), course of stroke (P=0.0001), the minimum distance of the anterio border of the lesion to the frontal pole(P=0.0137), the ratio of the distance of the anterio border of the lesion to the frontal pole and the distance from the frontal to occipital pole(P=0.004). Multiple stepwise Logistic regression indicated that the chiefly related risk factors associated with the occurrenc of PSD were economic status(OR=1.841), activities of daily living (ADL) (OR=1.043), number of lesion (OR=0.770), course of stroke(OR=0.833), family relation (OR=8.001), the ratio of the distance of the anterio border of the lesion to the frontal pole and the distance from the frontal to occipital pole (OR=11.108). The occurrenc of PSD were not associated with age, culture lever, occupation, medical insurance, type of stroke, lesion volume, side of lesion, lesion stratum. In test-retest reliability research, the Spearman rank correlation coefficient of 11 item problems of the first segment range from 0.81 to 0.96(P=0.0001), the Spearman rank correlation coefficient of 9 item problem of the second segment range from 0.70 to 0.93(P= 0.0001), test-retest reliability were better. Cronbach's alpha coefficient of the first segment was 0.845, Cronbach's alpha coefficient of the second segment was 0.803, internal consistency of MMADD were better.
