Objective : To investigate the incidence and clinical features of post-stroke depression in patients with acute stroke in Longshan county.To analyze and explore the influencing factors of post-stroke depression and to control the intervention factors as early as possible to provide the basis for optimizing the comprehensive treatment of post-stroke depression.Methods: This study collected acute stroke in the neurology department of Longshan County People's Hospital from November 2018 to May 2019,cases selected according to inclusion criteria,were divided into depression group and non-depression group by ?the manual of diagnosis and statistics of mental disorders-the fifth edition DSM-5 ?,and then the degree of depression was assessed according to hamilton 17 depression scales(Hamilton Depression Scale HAMD-17).The basic demographic data(gender,age,education,marital status),family function(family care index),daily living ability(Barthel index),disease history(stroke type,stroke focal location,number of stroke lesions,NIHSS score)and related medical history(hypertension,diabetes,hyperlipidemia,smoking,alcohol consumption)were recorded by establishing family health records for all patients.The obtained data were analyzed by SSPS 23.0 software description,the measurement and counting data were tested by t-test and chi-square test respectively,P<0.05 said the results were statistically significant difference.The above meaningfulsingle factors were divided into two categories Logistic regression analysis,and the independent risk factors post-stroke depression.Results : A total of 139 patients with acute stroke were enrolled in this study.The number of depression group and non-depression group were 43 and96,respectively.The incidence of post-stroke depression was 30.9%,of which 28 were mild depression(65.1%),11 were moderate depression(39.2%)and 4 were severe depression(9.3%).There was no significant difference in age and education between depression group and non-depression group(P>0.05).Post-stroke depression is more likely in women,divorced or widowed stroke patients who lack family care and are frequently affected with severe neurological deficits(P < 0.05).There were significant differences in hypertension and diabetes between the two groups(P<0.05).But there was no significant difference in smoking history,drinking history,hyperlipidemia,lesion location and stroke type between the two groups(P<0.05).Multivariate Logistic regression analysis revealed that Male(OR=0.152,POR=0.001),single lesion(OR=0.243,POR=0.023)were protective factors,hypertension(OR=7.085,P=0.008),neurological deficits(OR=1.496,P=0.001)were independent risk factors for post-stroke depression.Conclusions:The incidence of post-stroke depression in Longshan County was 30.9%,with mild-moderate depression as the main clinicalmanifestation,generally depression and loss of interest as the main clinical manifestation.post-stroke depression was caused by a multi-factors synthesis,female,low family care,divorce,widowhood,multiple lesions,serious neurological deficit is more likely to prone to post-stroke depression;hypertension,female,neurological deficit and multiple lesions are independent risk factors for post-stroke depression,early active prevention and treatment of their risk factors is crucial.
