ObjectiveThe aim of medical licensing examination is to figure out whether the participants obtained the ability to perform clinical and medical activities. Level of the examinations can reflect the knowledge and technique of the participants in some degree. To improve the medical licensing examination in our country, reform the model of medical education, and foster qualified practitioners, we need to study and make use of advanced experiences and methods in medical examinations of other countries. The system of medical licensing examination in Japan had an earlier beginning, and by far it has become an integral and systemic model. Moreover, with the development of medical sciences, the system in Japan can renew itself with new characteristics of the era. The educational system in our country is very similar with that in Japan, so we can analyze and compare the educational systems. Through the simulated testing, we find out the similarities and differences in these two educational systems, analyze the related factors, compare the characteristics of medical licensing examinations in China and Japan, and evaluate the medical ability and knowledge of students. We provide valuable reference to improve the system of medical examination and reform the model of medical education.MethodsThe students at the department of clinical medicine educated mainly in Japanese, who were in their final school year, were included in our study. We selected testing papers of the 94th Japan medical licensing examination in 2000 and testing papers of the 2nd China medical licensing examination. Simulated examinations were taken as the true examinations. At the same time, questionnaires were performed to investigate the medical licensing examination. The results of examinations were analyzed by using special statistical software.Results(1) In simulated tests, score from testing papers of China was significantly higher than that of Japan. The significance was statistical.(2) There was no difference in testing scores from male students and that from female students in 2 simulated tests.(3) No relationship was found between score from testing papers of Japan and score of Japanese testing as a language. However, level of Japanese as a language in special fields had effects on the score from testing papers of Japan of this study.(4) Scores from the 2 simulated tests were all related with the average score in true examinations of the students. Linear relationship was found between the score from testing papers of Japan of this study and the score of true examinations in clinical education. The relationship was different when the testing papers of Japan were classified as special fields. Scores of simulated tests in basic medical sciences, basic clinical medicine, internal medicine, and surgery were significantly related with those scores from the true tests. However, scores of simulated tests in public health, gynecology, and pediatrics had no relationship with the scores from the true tests. Scores of simulated tests with testing papers of China had no relationship with the scores of true tests when the testing papers were classified as special fields.(5) In simulated test with papers of China, the total grasping degree and grasping degree in special fields were very near to those of sample from the whole country.( 6) The investigating results of questionnaires showed that, the difficulty of testing papers of Japan that the students evaluated was identical to the results of the simulated tests. Most students thought the difficulty in special fields with score relationship was moderate, and the difficulty in those without score relationship was high.(7) The investigating results of questionnaires about recognizing on testing papers of public health showed that, most students recognized the importance of-4 ?knowledge of public health, and it was necessary to become the content of testing.(8) Most students' answers on questionnaires showed that the tests of our study can truly reflect the ability and gras...
