Limb-salvage Treatment For Malignant Bone Tumors In 68 Patients

Objective: To assess the result of 68 cases of the limb salvage treatment, and discuss the problem existed in the treatment.Methods: We treated 68 patients suffered malignant bone tumors from 1990 to 2002 by limb salvage surgery. There were 40 males and 28 females, the mean age was 25.37 years; The treated sites were knee periphery (46), proximal femur (9), middle piece of femur (2), proximal humerus (6), pelvis(3 ),fibular(1), navicular bone (1) ;The histologic diagnoses were osteosarcoma (41), chondrosarcoma (6), malignant fibrous histiocytoma(6), giant cell tumor of bone(4), metastasis adenocarcinoma(3), Ewing's sarcoma (1), and others(7); The stage of tumor was II b in 49 patients, II a in 12, and I b in 1 , All the patients received chemotherapy and limb salvage surgery.Results: At a follow-up ranging from 2 to 85 months (median, 23.5 months), 16 patients died of lung metastasis, and local relapse in 12, infection in 6, foot drop in 2, thenar anaesthesia in 1, fracture in 2, cutis necrosis in 1 .Functional evaluation of theaffected limbs was excellent or good for 39 patients, fair for 27, and poor for 2. Most of the patients were satisfy with the results.Conclusion: The 5-years-overall survival and disease-free-survival of our patients was relative low, the reason may consist of: (1)The individual chemotherapy was not normative enough. (2) We haven't established a objective system to evaluate the histological response to chemotherapy ofosteosarcoma.(3)Some patients didn't completed the whole course7 of treatment.(4)In the early years ,there were few data of radiological imaging available, it was difficult to grade the osteosarcoma exactly. ( 5 ) In some cases, it is hard to remove the mass with wide surgical margins . we will seek a more effective multidisciplinary treatment including immunotherapy , genetherapy, biotherapy and thermotherapy.
