Objective Atherosclerotic thrombotic cerebral infarction (ATCI) is avery common disease with high morbidity and mortality in clinical practice. So prohibition from atherosclerosis remains a challenge. Recent studies have found that there is a glycoprotein called paraoxonase-1 (PON 1) in serum, which can protect atherosclerosis by many means. Consequently serum PON1 activity is negatively correlated with the development of atherosclerosis. Moreover, some investigators report that PON1 Q/R polymorphism may also affect atherosclerosis. The present study wants to reveal the relationship among PON 1 Q/R polymorphism, serum PON1 activity and atherosclerosis in Chinese Han ethnic group and to research the function and mechanism of PON 1 in atherosclerosis.Methods We conducted a case-control study of 48 hospital cases withATCI (male 28, female 20, mean age 63.2+11.3 years, age range 45-80 years) and randomly selected healthy control subjects (male 34, female 21, mean age 59.8+8.7 years, age range 45~80 years) .The PON1 Q/R polymorphism was evaluated by polymerase chain reaction and restriction fragment length polymorphism (PCR-RFLP) , and serum PON1 activity was determined using phenyl acetate as substrate by measuring the increase rate in the absorbance at 207 nm due to formation of carbolic acid. Ox-LDL was analyzed by emzyme-linked immunosorbent assay (ELISA) and TC, TG, FBS was estimated by autoanalyzer.Results (DThe distribution of the PON1 Q/R polymorphism in the two groups was QQ genotype: 8%(4/48), 16%(9/55); QR genotype: 56%(27/48),53%(29/55); RR genotype: 35%(17/48), 31%(17/55); QR+RR: 92%(44/48), 84%(46/55); QQ+QR: 65%(31/48), 69%(38/55), respectively. There was no significant difference between them. In the above two groups allele frequency was Q: 0.37 and 0.43; R:0.63 and 0.57, respectively. There was no significant difference between them too. (2) Serum PONl activity in the hospital cases was significantly lower than control subjects (74.41 ?8.85 ug/ml, 113.65+26.64 ug/ml, P=0.000).(3) Senility, smoking, hypertension and TC, LDL, ox-LDL made serum PONl activity decrease. (4)There was no significant difference in TC, TG and serum PONl activity among defferent genotypes.(5) Logistic regression analysis revealed that smoking, hypertension and low serum PONl activity were the independent risk factors of ATCI.Conclusions (DThe present study suggests that There is no significant association between PONl Q/R polymorphism and ATCI in Chinese Han ethnic group. (2) PONl can preserve LDL from oxidation, therefore prevent the development of atherosclerosis. (3)Smoking, hypertension and low serum PONl activity are the independent risk factors of ATCI.
