Pregnancy Induced Hypertension (PIH) is a common complication of pregnancy and is associated with significant fetal and maternal morbidity and mortality, so predictive and prophylactic treatment earlier is important. We study the hemodynamics mechanism of PIH, the correlation of different parameters and the law of dynamic changes. On the basis of these researches, we developed a new way using the dynamic trend of cardiovascular parameters to predict PIH. Record and analyze the changes of cardiovascular parameters in 129 pregnant woman during the whole gestation from 10-13 weeks. Using double-blind method to study the mechanism of PIH. We found the hemodynmics is heterogenous, 2 women developed high cardiac output with the normal or low peripheral resistance; the others developed the high- peripheral output with the normal or low cardiac output. There is an example who experienced the crossover from the high cardiac output to high peripheral resistance, it resembles the process of chronic hypertension. According to the relationship of the blood pressure,cardiac output and peripheral resistance, we found the disorder of the negative feedback in blood pressure come forth earlier in PIH than in the normal pregnancy. Compared with other hemodynamic parameters the abnormity of peripheral resistance appears earlier and more evident. Just by the variance of it we forward a new method using the dynamic trend of cardivascular parameters to identify PIH. Using the dynamics trend of peripheral resistance to predict PIH in the 129 pregnant woman. We found that in 20k?week gestation age the PPV (positive predictive value) is 41.7%, in 24~ week the PPV is 66.7%. Compared with the static method and MAP (mean arterial pressure) the predict value increased, and the moment to predict is at least 4 weeks earlier than when the clinical characters appear. So the dynamic trend method is valuable to predict PIH and is feasible for using in clinic.
