Research Of Pulse Waveform With Gestational Age Changes Of Pregnant Women With Hypertensive Disorder Complicating Pregnancy

Hypertension disorder complicating pregnancy（HDCP）is a group of diseases with elevated blood pressure and pregnancy,the incidence rate in China is about 5%to 12%,the incidence of it is hidden,diagnosis lags,clinical symptoms appear late,all points above bring greater difficulties in prevention and treatment.With the increase of gestational age,pregnant women’s cardiovascular function will change,and pulse wave contains extremely rich physiological and pathological information of cardiovascular system.The pathogenesis of HDCP is the spasm of systemic small vessels,although the clinical symptoms of pregnant women appear relatively late,but the cardiovascular function has been different from the normal pregnancy of pregnant women,therefore,it is very important to study the changes of pulse wave waveform with gestational age of pregnant women with HDCP.In this research,the PowerLab data acquisition and analysis system,the radial artery pressure sensor and the finger clip type transmission photoelectric sensor were used to collect the data of radial pulse wave and photoplethysmography（PPG）pulse wave of pregnant women.The MATLAB software was used to write the program to achieve the pulse wave waveform pretreatment,draw the oscillogram of pulse wave waveform changed with the gestational age,and calculate the heart rate,the pulse peak time,the dicrotic notch time,the K value,the K₁ value and the K₂ value of radial pulse and PPG.The aim of this study was to inveatigate the changes of radial wave and PPG waveforms with gestational age,and the SPSS statistical analysis software was used to analyze the parameters of radial wave and PPG,blood pressure and heart rate of normal and HDCP pregnant women in different groups of the geatational age.Though the study we found,in the 24-27 weeks of gestation,the dicrotic notch time of radial pulse wave and PPG pulse wave of the pregnant women with HDCP was significantly higher than that in normal group（P<0.05）,but the K₁ value and K₂value of PPG pulse wave in HDCP group was significantly lower than that in normal group.There were no significant differences between the two groups in the other waveform parameters of radial pulse wave and PPG pulse wave.Indicating that during this period,the cardiovascular and microcirculation became worse,the resistance of vascular increased and the elastic became weakened of the pregnant women with HDCP.In addition,the systolic blood pressure of normal pregnant women changed between 105mm Hg and 115mmHg,the diastolic blood pressure varied between67mmHg and 74mmHg,and the heart rate varied from 85bpm to 95bpm.In the course of HDCP,the systolic blood pressure of the pregnant women with HDCP changed between 116mmHg and 123mmHg,the diastolic blood pressure changed between73mmHg and 78mmHg,and the heart rate varied from 85bpm to 100bpm.The heart rate of pregnant women with HDCP was significantly higher than that in the normal group during the 24-27 weeks of gestation,and it was suggested that the pregnant women with HDCP needed to accelerate the heart beat to improve the heart rate to satisfy the basic needs of their own and the fetus compared to the normal pregnant women during this period.According to the above conclusions,there was the biggest difference of cardiovascular and microcirculation function between the two groups in the 24-27weeks of gestation,which was mainly manifested in the dicrotic notch time of the pregnant women with HDCP was significantly longer than that in the normal group（P<0.05）,and during this time,the pregnant women were not have any corresponding clinical symptoms.In this study,the pregnant women were diagnosed with HDCP in the 39±2 weeks of gestation.The findings of this study were 12-15 weeks earlier than the onset time.The results confirmed the significance of this research.
