Analysis Of The Clinical Application Of Tang Xianping 's Physician In The Treatment Of Adult Steele Disease Based On TCM Inheritance System

BackgroundAdult-onset Still’s disease (AOSD) is a rare inflammatory condition of unknown etiology characterized by high spiking fever, salmon-pink evanescent rash, sore throat ,arthralgia, arthritis, and accompanied by peripheral blood leukocytes and granulocytes increased, and hepatosplenomegaly and other systems involved in the clinical syndrome. The patients have suffered tremendous pain both in mental and physical because of the easy recurrence of the disease. The disease has been called " Allergic and sepsis", and known as "Adult-onset Still’s disease" until 1989.The main and basic therapy of Western medicine for AOSD are Non-Steroid Anti-Inflammatory Drugs(NSAIDs)、Glucocorticoi (GC) and Disease Modifying Anti-Rheumatic Drugs(DMARDs),however, the long-term use of the drugs causes serious side effects, some of which can be deadly. There are a small number of patients died from sepsis, tuberculosis and acute liverfailure during the course of hormone therapy. The introduction and application of biological agents in recent years might benefit the patients with AOSD, but the uncertain therapeutic effects and the expensive price make the new drugs difficult to popularize.The Chinese medicine (TCM) may play an important role in the process of the treatment for this disease. Although the etiology and pathogenesis of the disease need a further research to make a accepted principle, TCM have a proven impact on fever、rash、joint pain and other symptoms of the disease and have a abundant clinical experience. Moreover, TCM can reduce negative effects brought by the long-term application of the western medicine, allow the reduction of GC going smoothly, and prevent recurrence. Therefore, it has important implications to actively explore a effective treatment for AOSD from TCM.Instructor Professor Tang Xianping researches in AOSD for years, during the course of therapy,he points that AOSD should belong to "warm disease" or "Bi Syndrome" according to its clinical characterisrics. Based on the theory of "evil fire and physical fire" in Classic of Internal Medicine, Professor Tang Xianping considers that the main pathogenesis of AOSD is Qi and Yin deficiency and wet-heat、sputum and blood statis, then obtains good results and rich clinical experience by using the treatment of Reinforcing Qi and Nourishing Yin、clearing heat and promoting diuresis, promoting blood circulation and removing obstruction in channels.ObjectiveAnalyzing the medication experience of Professor Tang Xianping on treating AOSD to make a summary of frequency of usage, regularity in drug combination and principles of formulating prescription, providing effective treatment to AOSD and reducing the pain of the patients.MethodsCollect the medical records used for 196 patients with AOSD, establish a database with TCM inheritance system(V2.0),by using the function of Data analyzing,to analyze the composing principles of prescriptions.Results539 prescriptions were filtered and analyzed for define the frequency of useage, regularity in drug combination and principles of formulating prescription. We got 20 core groups and 10 new prescriptions, and these were conformed as the core herbs for treating AOSD by Professor Tang Xianping.ConclusionProfessor Tang Xianping’s prescriptions has outstanding characteristics in treating AOSD. The consequences were obtained by the TCM inheritance system and the professor’s experience is generally accordant. The system can be used to analyze and summarize the experiences of traditional Chinese physician.
