| Objective By using the methods of literature research,TCM academic research,comparative research,etc.,through the network literature retrieval system,major libraries in Guangzhou,interviewing the academic heirs or descendants of famous old TCM doctors,and visiting and studying with famous old TCM doctors,the academic thoughts,diagnosis and treatment characteristics and clinical experience of famous TCM doctors in Lingnan were collected on a large scale,and the incidence characteristics of menstrual diseases and the diagnosis and treatment rules of famous doctors in Lingnan were explored.This paper presents the origin of academic thoughts and the integration of characteristic experiences of Lingnan gynecologists in treating menstrual diseases,which provides a basis for clinical inheritance and application of academic thoughts and clinical experience of Lingnan Gynecology School.On this basis,the clinical efficacy of one famous doctor’s experience prescription was preliminarily verified by participatory research method,which provided evidence-based basis for the clinical efficacy of Lingnan traditional Chinese medicine gynecology academic thought.It provides guidance for future generations to learn Lingnan gynecology,and lays a certain foundation for giving full play to the advantages of Lingnan gynecology and inheriting and developing Lingnan gynecology.Methods This research mainly collects the academic background,academic thoughts,clinical experience and effective medical records of famous gynecologists in Lingnan through extensive collection of relevant historical records, published papers,medical records,teacher-following records of famous doctors’ disciples,and other records of famous doctors in other ways.Through comparative research methods,the characteristics and laws of famous doctors in Lingnan for the diagnosis and treatment of menstrual diseases are summarized and sorted out,forming "Lingnan Chinese Medicine Gynecology Treatment of Menstrual Diseases" At the same time,the diagnosis and treatment characteristics of famous gynecologists in Lingnan Chinese medicine for treating menstrual diseases were sorted out and studied.The academic thoughts,speculative characteristics,clinical experiences,prescription characteristics and typical medical cases of contemporary famous gynecologists in Lingnan Chinese medicine were studied by means of literature collection,self-summary of famous Chinese medicine practitioners,interviews with famous experts,disciples and follow-up study,and various traditional and modern research methods and comprehensive technical means,and their common laws and characteristics were summarized.Based on the above summary of clinical experience and characteristics of Lingnan famous gynecologist in treating menstrual diseases,an empirical prescription of Lingnan famous gynecologist Li Liyun was selected,and participatory clinical research was adopted,starting with prescription and treatment,and a representative research scheme was formed to evaluate the clinical efficacy,providing evidence-based basis for the clinical efficacy of Lingnan traditional Chinese gynecology academic thought.Results In the development of Lingnan medicine,the development of the gynecological schools of traditional Chinese medicine in Lingnan was deeply influenced by the historical precipitation of Lingnan culture.The formation factors of Lingnan TCM gynecology include geographical,economic and cultural factors,and physical factors.The composition of Lingnan TCM gynecology school is represented by the Roche Gynecology and Cai ’ s Gynecology schools.It also includes Lingnan physicians who specialize in gynecological diseases and famous experts who also treat gynecological diseases.The academic characteristics of Lingnan traditional Chinese medicine gynecology school in the treatment of menstrual diseases:(1)Inherit and carry forward traditional Chinese medicine,innovate traditional Chinese medicine theories,and cultivate several generations of famous doctors in Lingnan;(2)All diseases are born in qi,pay attention to regulating qi machine;(3)Adjust measures to local conditions,pay attention to damp and heat disease,and protect qi and yin;Pulse diagnosis;(5)simultaneous treatment of mind and body,regulating emotions,and emphasizing emotion treatment;(6)regulating kidney and spleen,paying equal attention to congenital and acquired;(7)medication lightly,good use of southern medicine and anthers;(8)combined with Lingnan diet culture,medicine and food are of the same origin,Pay attention to medicated diet.In this study,the No.8 Chinese medicine compound of No.8 kidney and spleen-tonifying method of the experienced prescription of Lingnan gynecological famous professor Li Liyun was selected to treat prolonged menstrual period(luteal insufficiency)with deficiency of both spleen and kidney.The randomized controlled method was used to compare with dydrogesterone tablets.The results are as follows : After treatment,the scores of the main syndromes of the two groups of patients with prolonged time were significantly decreased,and there was no statistical difference between the groups(P>0.05),indicating that the treatment effects of the two groups were equivalent;the scores of TCM syndromes of the two groups after treatment were more obvious than those before treatment Decrease,the comparison between groups is statistically significant(P<0.05),indicating that the traditional Chinese medicine group is more effective in improving patients’ traditional Chinese medicine symptoms.After three menstrual cycles,the HPS scores of the two groups were significantly higher than those before the treatment,but the difference between the two groups after treatment was not statistically significant,indicating that the two groups had the same effect.The duration of the basal body temperature and hyperthermia phase between the two groups was prolonged after treatment,but there was no statistically significant difference between the groups after treatment(P>0.05),indicating that the two groups had similar effects in prolonging the luteal phase.Serum progesterone levels in the middle luteal phase(BBT increased for 6-8 days)in the two groups were significantly increased after treatment(P<0.05),and the difference was statistically significant,indicating that the two treatments may both increase serum progesterone in the middle luteal phase Value and improve corpus luteum function.However,due to the comparison of the serum progesterone levels in the mid-luteal phase between the two groups before and after treatment,the differences were not statistically significant,and the groups were not comparable.Therefore,the effectiveness of the traditional Chinese medicine group in increasing the serum progesterone levels in the mid-luteal phase cannot be proved.Conclusion The uniqueness of Lingnan’s climate and geography has caused Lingnan’s disease characteristics and physiques of residents to be different from other regions in my country.For example,external evils often invade the human body with damp evil,and patients with menstrual diseases also have many qi deficiency,yin deficiency,and phlegm damp constitution;There is also the food culture habit of using local Chinese herbal medicine to cook herbal tea and soup.Therefore,medicated diet has become a major feature of Lingnan Traditional Chinese Medicine Gynecology.Inheritance characteristics of Lingnan gynecology school: Taking the representative South China Sea Roche gynecology as an example,its menstruation regulation features are:(1)Advocating Jingyue,regulating menstruation first emphasizes Yin and Yang;(2)The spleen and kidney are based,qi and blood are used;Replenishment degree;(4)Treatment pays attention to the difference of regional climate;(5)Make good use of southern medicine,the medicine is calm;(6)Emotional treatment is emphasized.Professor Luo Yuankai inherited and carried forward,and he was devoted to China and the West,and proposed the concept of female hypothalamus-pituitary-ovary-uterine gonadal axis.This is a major breakthrough in Lingnan TCM gynecology theory.Roche’s successors,Ouyang Huiqing,Li Liyun,Luo Songping,Wang Xiaoyun,etc.,inherited Roche’s academic thoughts,and each has its own development and innovation.Ouyang Huiqing is committed to the application of the method of invigorating the kidney and promoting blood circulation in the field of gynecology for common and refractory diseases,and her scientific research has achieved fruitful results.Li Liyun enriched the kidney-tonifying method and proposed the eight classic kidney-tonifying methods,which enriched the experience of the kidney-tonifying method. Based on the unity of nature and man,Luo Songping put forward an innovative viewpoint that the menstrual cycle is synchronized with the lunar rhythm.After years of clinical practice and scientific research,Wang Xiaoyun has formed a distinctive TCM academic thought of "Mind and Body Treating Simultaneously",and innovated emotional therapy that uses emotion to win.At the same time,the inheritor of Lingnan Gynecology School created a school studio for the research of Lingnan Gynecology School of Traditional Chinese Medicine,cultivated talents,and made contributions to Lingnan Gynecology School.The inheritance of Chaoshan Cai ’ s Gynecology is based on family traditions,except for its100-year-old curative effect.Modern and modern famous doctors Cai Yanggao and Cai Chunchen worked hard to inherit good family traditions and medical ethics,Chong Zhongjing’s doctrine,emphasized the spleen and kidney,learned from others’ strengths,paid attention to the ancestor’s prescriptions and folk prescriptions,and was innovative.In modern times,Professor Cai Miaoshan inherited the previous experience,combined with the absorption of Chinese medicine teaching in colleges and universities,and developed Cai’s gynecology.In terms of clinical verification,the No.8 Jianpi Bushen Decoction of Professor Li Liyun’s Experience is effective for patients with spleen-kidney deficiency type menstrual prolongation(caused by luteal insufficiency),and its curative effect is similar to that of dydrogesterone tablets in improving luteal insufficiency type menstrual prolongation.It is equivalent,and it is better than dydrogesterone in improving the symptoms of patients with spleen and kidney deficiency in TCM.This empirical formula has clinical feasibility and repeatability. |
