Wang Yanheng, Chief Physician, Summed Up The Academic Experience Of Yishen Jiannao Method And Clinical Research On The Treatment Of Prodromal Schizophrenia With Yishen And Liver Method

WANG Yan-heng,the national old traditional Chinese medicine(TCM)doctor,was the tutor of academic experience inheritance of national old TCM doctor of the state administration of TCM.He,worked in Beijing Anding hospital,Capital Medical University for 50 years,is the only one old TCM doctor in mental diseases in China.He had unique theoretical knowledge and clinical experience in diagnosis and treatment of mental diseases.Nourishing Kidney and Brain is the key of WANG Yan-heng' academic thought in diagnosis and treatment of mental diseases.This idea would be studied and discussed in this article.In part 1,I had inherited the academic thought of Nourishing Kidney and Brain of WANG Yan-heng,and following this academic thought,I created a new model of different symptoms having different syndrome based on my 30 years experience of preclinic and clinic of Chinese and West medicine in psychiatry,that's,mental symptoms were the core of different symptoms based on the theory of Wu Shen Zang.Po of Lung was underlined in this new model in diagnosis and treatment of mental diseases.Nourishing Po of Lung could inhibit Hun of Liver,calm Shen of Heart,quiet down Zhi of Kidney and withdraw Yi of Spleen to treat mental disease.This academic thought developed the idea of calming down the activity of brain to treat mental disease,which was one of the part of WANG Yan-heng'academic experience.The new model of different symptoms having different syndrome formed the characteristic theory and clinic and had good clinical effect.In part 2,the article summarizes Chinese and Western medicine psychiatric clinical experience of application of the Strengthening lung-po theory in the treatment of mental illness.The artical studies theoretical basis and clinical practice of syndrome differentiation by Wu Shen Zang,how Strengthening lung-po could tranquilize liver-hun,heart-shen,kidney-zhi and spleen-yi.It especially emphasizes the importance of Strengthening lung-po in syndrome differentiation and treatment of mental illness.The five zang-shen as the core of syndrome differentiation has its own superiority,the method is worth more exploring and summarizing than syndrome differentiation by body symptoms.This further exerts Master WANG Yan-heng's academic thought of nourishing Kidney and Brain,and protrudes the academic idea of "Calmness is important."In part 3,first,the paper summarizes the research progress of prodromal schizophrenia.Second,the article statistically analyzed syndrome element by the data from 105 cases of prodromal schizophrenia.Results:Lax attention,red tongue,string pulse on left,delusion,tension,fear,auditory hallucination,thin-white tongue coating,thready pulse on chi,slippery pulse on right,irritability,solitude without desire,insomnia,chuckle,mumbling,folly,rapid pulse are common symptoms of TCM;Kidney and Liver are main syndrome elements of disease location,syndrome element of disease character is given priority to Fire,and Yin deficiency as the auxiliary pole;Yin deficiency and negative symptoms,negative symptoms and Kidney,Heat and positive symptoms,positive symptoms and Liver have more closely correlation.Conclusion:The core pathogenesis of prodromal schizophrenia is the syndrome of Kidney deficiency and Liver hyperactivity,based on Yin deficiency of Kidney,and exerted as the fire of Liver and Kidney;therapeutic treatment is tonifying Kidney and calming Liver,soothing the nerves of Liver's Hun and Kidney's Zhi.Last part,the effect of Yi Shen Pin Gan Fang-a Chinese Herb Formula in treating the Ultra High Risk for Psychosis population was investigated using double blind randomized controlled study compared with aripiprazole.Results:For GAF,PANSS and SIPS,there was no difference between two groups(F=0.04,0.05,0.15,P>0.05),however,there was significant improvement since 4th week till 12th week in each group compared with baseline(F=27.16,59.91,55.92,0.05,0.15,P<0.001).For the Trail Making test,there is no difference between and intra groups(P>0.05);For the Digital Symbol test,there is near significant difference in Chinese Medicine Treatment group(CMT group)compared with baseline(P=0.06),and no significant difference in Aripiprazole Treatment group(AT group)(P>0.05);For Verbal Learning test,both CMT group and AT group have significant difference compared with baseline(P<0.05);For Visual Memory test and Stroop-(colour/word)test,there is significant difference in CMT group compared with baseline(P<0.05),and no significant difference in AT group(P>0.05);but in Stroop-word test,there is significant difference in AT group compared with baseline(P<0.05),and no significant difference in CMT group(P>0.05);For CPT,there is significant difference in CMT group compared with baseline(P<0.01),and no significant difference in AT group(P>0.05).No obvious side effects were found in Yi Shen Pin Gan Fang treatment group.The result of this study proved that the prescription of Yi Shen Pin Gan Fang was effective for prodromal stage,and deficiency Kidney and over-active Liver was the principal pathogenesis of prodromal stage,so how to treat schizophrenia in the high-risk groups by TCM was established,which was a new interfering method for schizophrenia in the high-risk groups with Chinese characteristics and originality in international area.This new method was on behalf of the essence of Chinese medicine prevention of disease and was a beneficial supplement for the method of nourishing Kidney and Brain of WANG Yan-heng' academic experience.This paper summarized and developed Master WANG Yan-heng's academic thoughts of nourishing Kidney and Brain,but there was a long gap between me and my teacher,which demanded continuing improvement for my knowledge level and clinical experience to inherit and develop the teacher's academic thought completely and deeply.
