| ObjectiveThe aim of this research is to study the rich academic thoughts and clinical experience in treatment of gynaecology disease and treatment rules of menopathy from Guangdong Province famous professor of TCM-Xiaoyun Wang, inherit traditional Chinese culture from her. And under the guidance of her academic thoughts, a clinical study of treatment of hypomenorrhea by using prescription of nourishing yin and smoothing the liver are designed and implemented.MethodsProfessor Wang Xiaoyun is a famous gynecology expert, has a unique academic thoughts and rich clinical experiences on the treatment of gynecological diseases. She is the director of gynecology in Second Clinical Medical College of Guangzhou University of TCM, the director of the Teaching and Research Section of Gynecology, Ph.D., postdoctoral co-supervisor, the Guangdong Province famous doctor of TCM, the fifth batch of teachers of the famous Chinese medicine experts academic experience the succession and degree guidance, the Distinguished Teacher in Guangdong Province, the national key clinical specialist academic leader, the head of key gynecological specialty assist leader hospital by the project from the State Administration of Traditional Chinese Medicine in "10th Five-Year Plan ", "11th Five-Year Plan", "12th Five-Year Plan", the national drug evaluation experts, the National Natural Science Foundation of peer review expert, the National Science and Technology Award accreditation expert.She had won "The first national outstanding female practitioners of TCM", "the first national outstanding students of inherit from TCM experience award, the national outstanding scientific and technological workers", the" good doctor of Guo Chunyuan type, "the national excellent Chinese medicine health messenger" and so on. She is the executive president of China National Medical Association Professional Committee of Department of gynaecology, vice president of the World Federation of Chinese Medicine Societies Professional Committee of Department of gynaecology, vice president of the World Federation of Chinese Medicine Societies Professional Committee of Department of reproductive medicine, the Standing Committee member of Chinese Acupuncture Association Specialized Committee of abdominal acupunctures, the Standing Committee member of China Association of Combination of Traditional Chinese Medicine and Western Medicine Department of Obstetrics and Gynecology, the Standing Committee member of World Federation of Chinese Medicine Societies ethics review committee, the chairman of Guangdong Province Combination of Traditional Chinese and Western medicine Department of Obstetrics and Gynecology, the vice-chairmen of Guangdong Province Chinese Medicine Societies Professional Committee of Department of gynaecology. She assumed 56 national, provincial and other levels scientific research, won 13 science and technology achievement awards, published 30 monographs, published 106 papers in academic journals (include 20 articles indexed by SCI, El, CA). She have trained more than 100 all levels professionals, include national academic heir, senior title and medium grade professionals, postdoctoral, and graduate, etc. for gynecology of traditional Chinese medicine.Professor Wang Xiaoyun studied conducting with "national medical master" Professor Lu Zhizheng, Guangdong province famous doctor of TCM Professor Li Liyun, initiator of abdominal needle Professor Bo Zhiyun, learned academic thoughts quintessence. She has accumulated rich experience in the treatment of gynecological diseases, especially in the treatment of menstrual disorders, menopausal syndrome, infertility and reproductive organs of tumor and other miscellaneous diseases. She has a high prestige in the academic circles. She kindly treat the patients, and try to use her own medicine to solve the suffering of the patients. Because of the noble medical ethics, she widely praised by society, enjoys a high reputation among the masses. In the past 3 years, under the guidance of tutor carefully, with the teacher to my clinic, ward, to discuss difficult cases, teacher interviews, I had a preliminary understanding to her origin of academic thoughts. A whole summary in the paper for thought and clinical experience of Professor Wang Xiaoyun are under the following several aspects.1. Attached great importance to the four diagnosis of traditional Chinese medicine, found important clues from tiny and identified difficult placeProfessor wang takes data collected from four clinics seriously and filters out the useful part in the treatment, finds the main contradiction of the disease, so as to choose the method of treatment of diease.2. Emphasized the role of internal organs physiology and pathology in gynecological diseasesKidney is the root of reproduction, professor Wang paid attention to the kidney-menstruation-Chong and Ren-uterus axis which related to menses. Kidney stores the essence included congenital and acquired ones and it can create blood which is the basis of menstrual material. So we can say that deficiency of the kidney is the reason for the menstrual abnormalities and infertility of female.Spleen corresponding to earth, controlling transport and transformation, providing the materiel basis of the acquired constitution, is the source of growth and development. Chong Mai belongs to Yang Ming which contains more Qi and blood. If the spleen and stomach healthy, refined essence enough, Qi and blood will be vigorous, Chong and Ren will be abundant, and menstruation, gestation, eutocia and galactopoiesis will be normal.Female easily depression, so we need ventilate lung, sooth nourish liver in order to regulate qi activity. Medical Zhen Zhuan ·Qi-Blood said, Qi and blood circulated in everyone’s body. Qi disharmonious without blood and blood doesn’t carry without qi. Women with blood to use, professor Wang considered, unobstructed of lung-qi was also important in the treatment of gynecological diseaseHeart is the major principal of visceras, it can relieve heart, anchore mind and stable qi. With the quickening pace of modern life, women take more work, life and family pressure, have great mental stress, therefore they easily engender diseases such as qi-stagnation, qi-regurgitating and qi-disorder, which lead to exuberant fire in heart and liver, failure of normal physiological function.The visceras related from each other, we should give consideration to them in the treatment.3. There is a unique academic thought in treating menopause before and after.There are many factors responsible for the syndrome before and after menopause, but they have a common root cause is kidney recession. Qi deficiency is the main cause of the onset of menopause. Professor Wang established a basic law to treat menopause syndrome is that accommodating yin and Yang, enhancing Qi and blood, Acquired congenital.4. Using TCM emotional therapy treats psychosomatic diseaseProfessor Wang has point of view of psychological to treat the development of radical changes of social and economic life bring more tension and pressure to women. Psychosocial factors play an important role in the development of gynecological diseases. Through theory of Unity of Mind and Body, Professor Wang applies TCM emotional therapy treat menopause syndrome have obtained satisfactory curative effects.5. Drugs and foods are homologous, eat foods in order to prevent diseaseProfessor Wang considers drugs and foods are homologous. She stresses that the treatment of disease while also focusing on dietary regulation. She stresses that paying more attention to health and prevention is greater than treatments.6. Get twice the result with half the effort at the same time using acupuncture and Chinese medicine treatmentProfessor Wang is skilled in Bo’s abdominal acupuncture, Balance Acupuncture, plum blossom needles, Dong Qi points, Sphygmopalpation Acupuncture, fire needle, etc. combined with traditional Chinese medicine to treat acute disease and miscellaneous diseases of gynecological. Her treatments make the doctors who came to study with her admire.In this clinical research,30 years of experiences of prescription of nourishing yin and smoothing the liver were summarized to design and implement to treat hypomenorrhea. The nourishing yin and smoothing the liver capsule has positive effect on hypomenorrhea which has type of syndrome of kidney deficiency and liver stagnation, and has obvious improvement effect on liver depression symptoms of patients.ResultAccording to the unified deployment and requirements of fifth batch of "Heritage Learning From the Heir" project from State Administration of Traditional Chinese Medicine and the Guangdong Provincial TCM Administration Bureau, I’m done 208 days of outpatient with the Professor Wang and ward round in three years,547 days independent clinical practice. Finished the Wang’s 60 complete medical record, and combined with my own experience and thinking, completed 36 month jour, completed 12 reading experience, published 1 article, publishing 1 article. Clinical research results show that the nourishing yin and smoothing the liver capsule treatment has an outstanding effect for 5 cases (12.5%), average effect for 16 cases (66.7%) and no effect for 3 cases (12.5%), giving a total efficacy of 84%. The Placebo shows a significant effect for 4 cases (15.4%) and no effect for 22 cases (84.6%), giving a total efficiency of 15.4%. The efficacies of the capsule and the comparing drug are compared by multiple categories, contingency tables ranking and testing, indicating a variation between them which is of great statistical significance. The result shows that the nourishing yin and smoothing the liver capsules for hypomenorrhea which has type of syndrome of kidney deficiency and liver stagnation have significant efficacy, in terms of both improve menstrual amount and TCM symptom.ConclusionThrough 3 years of learning with the teacher, this article summarized Professor Wang Xiaoyun’s academic ideological origins, academic experience and the diagnosis and treatment of gynecological diseases related rules. Clinical studies were carried out, and prescription of nourishing kidney yin and smoothing the liver in treatment to hypomenorrhea was used under the guidance of the Professor Wang. Studies show that nourishing yin and smoothing the liver capsule has positive effect on hypomenorrhea, and has obvious improvement effect on liver depression symptoms of patients. |
