Objective: A prospective,randomized,parallel controlled clinical research was performed in patients with post-infectious cough who were in wind-phlegm evil invading the lung syndrome.Compared differences of symptoms,reduce time of cough,recurrence of cough,LCQ and HARQ scale integral,SP and IL-6 number of sputum between two groups;Observed SP,IL-6,TNF-a levels in serum of rat,pathology of lung tissue and the influence of.COX-2,EP3,TRPV1 m RNA level and TRPV1 Channel Protein expression in lung tissue and dorsal root ganglion.To explore the mechanism of Chanqin Granule in the treatment of PIC.Methods:PART I Research on the impact of Chanqin grannule's treating airway neurogenic inflammation of the patient with PIC85 patients were selected from TCM traditional department and Respiration department of Shuguang hospital and TCM traditional department and Respiration of department of Pudong New Area Geriatric Hospital with PIC of wind-phlegm evil invading the lung syndrome.The patients were randomly divided into the Chanqin grannule group of 43 cases and Compound Methoxyphenamine Capsules group of 42 cases,treatment for 2 weeks.By comparing the reduce time of cough,recurrence of cough,LCQ and HARQ scale measurement,sputum supernatant SP,IL-6 number between the two groups,observed the effect of Chanqin granule on symptoms,quality of life in patients with PIC in the intervention.PART II Research on the mechanism of Chanqin grannule's effect on the airway neurogenic inflammation in rats36 SD rats were randomly divided into 6 groups,blank group,airway neurogenic inflammation model group,Compound Methoxyphenamine Capsules group and Chanqin grannule'low,medium and high dose groups,six in each group.Observed lung tissue pathological changes by HE dyeing;detected the serum SP,IL-6 and TNF-?content by ELISA method;Detected COX-2,EP3 and TRPV1 in lung tissue and dorsal root ganglion by RT-PCR method;detected TRPV1 protein expression by western-blot method.Observed the Chanqin grannule on rats with airway neurogenic inflammation in the intervention effect.Results:Part I1.Signs and symptoms: Comparisons of cough,itching,sputum,shortness of breath and cachexia before and after treatment in the two groups,the difference was statistically significant(P<0.01),indicating that the two groups could alleviate those symptoms;The efficacy scores of Chanqin grannule groups in two main symptoms of cough,pharyngeal itching efficacy scores and Compound Methoxyphenamine Capsules group were statistically difference(P<0.05),indicating that Chanqin grannule's curative effect on relieving cough and itching symptoms was better than Compound Methoxyphenamine Capsules.2.Time to ease:Chanqin grannule used shorter time than Compound Methoxyphenamine Capsules to reduce cough(P<0.05).3.LCQ scale:The scores of Chanqin grannul groups in the region of the Physiology,psychology,society and LCQ total score were higher than Compound Methoxyphenamine Capsules group,overall curative effect were superior to the Compound Methoxyphenamine Capsules(P<0.05).4.HARQ scale: The scores of Chanqin grannul groups in HARQ total score were no significant difference than Compound Methoxyphenamine Capsules group(P>0.05).5.The disease curative effect:Disease curative effect evaluation of patients between the two groups had statistical significance(P<0.05),Chanqin grannule curative effect was better than that of Compound Methoxyphenamine Capsules group.6.Induced sputum SP value: The SP values of induced sputum in Chanqin granule group and Compound Methoxyphenamine Capsules group were significantly different before and after treatment(P<0.001);but there was no significant difference between the two groups(P>0.05).Both of Chanqin granule and Compound Methoxyphenamine Capsules can reduce induced sputum SP value and relieve neurogenic inflammation.7.Induced sputum IL-6 value: The IL-6 values of induced sputum in Chanqin granule group and Compound Methoxyphenamine Capsules group were significantly different before and after treatment(P<0.001);but there was no significant difference between the two groups(P>0.05).indicating that both of Chanqin granule and Compound Methoxyphenamine Capsules can reduce induced sputum IL-6 value and relieve airway inflammation.Part II1.Cough frequency and latency:After treatment,there was significant difference between all treatment groups and model group,suggesting that both Chanqin Granule and Compound Methoxyphenamine Capsules can reduce the cough frequency of rats to a certain extent;After treatment,the cough latency of all treatment groups was prolonged,the difference was statistically significant between all treatment groups and model group,suggesting that both Chanqin Granule and Compound Methoxyphenamine Capsules can alleviate cough induced by cough provocation test and reducing cough sensitivity to a certain extent.2.SP value in serum:After treatment,the SP contents of the groups of Chanqin grannule low dose group,middle dose group,high dose group and Compound Methoxyphenamine Capsules compared with model group decreased had statistical difference(P<0.05).There was no statistical difference in each treatment groups(P>0.05).This result indicated that all treatment groups can reduce SP value in serum.3.IL-6 value in serum:The IL-6 content in serum of Chanqin granule in middle dose group,high dose group and Compound Methoxyphenamine Capsules group decreased significantly(P<0.01),indicating that medium and high doses of Chanqin Granule and Compound Methoxyphenamine Capsules can reduce IL-6 value in serum.4.TNF-? value in serum:The TNF-? contents in serum of Chanqin granule in middle dose group,high dose group and Compound Methoxyphenamine Capsules group decreased significantl(P<0.05,P<0.01),Compound Methoxyphenamine Capsules group was higher than other groups with statistical difference(P<0.01),suggesting that medium and high doses of Chanqin Granule and Compound Methoxyphenamine Capsules can reduce TNF-? value in serum.5.COX-2 m RNA expression in lung tissue and dorsal root ganglion:After treatment,the COX-2 m RNA levels of the high dose group of Chanqin grannule group in lung tissue and dorsal root ganglion were lower than the model group(P<0.05),suggesting that the high dose group of Chanqin Granule could decrease the expression of COX-2m RNA in lung tissue and dorsal root ganglion..6.EP3 m RNA expression in lung tissue and dorsal root ganglion: After treatment,EP3 m RNA levels in lung tissue and dorsal root ganglion in each treatment group was lower than that of model group,but there was no significant difference(P>0.05)..7.TRPV1 m RNA expression in lung tissue and dorsal root ganglion:TRPV1m RNA levels in lung tissue in all treatment groups were lower than the model group(P< 0.01),suggesting that all treatment groups can reduce TRPV1 m RNA expression;TRPV1m RNA levels in dorsal root ganglion in each Chanqin granule group were lower than the model group and Compound Methoxyphenamine Capsules group(P<0.05,P< 0.01).8.TRPV1 protein expression in lung tissue and dorsal root ganglion:After treatment,TRPV1 protein expression in lung tissue and dorsal root ganglion in each treatment group was lower than the model group(P<0.01),suggesting that all treatment groups can reduce TRPV1 m RNA expression in lung tissue and dorsal root ganglion.Conclusion:1.The Chanqin grannule could effectively improve the symptoms of PIC,elevated LCQ and HARQ scale scores,reduced neurogenic inflammation and curative effect affirmation.2.Chanqin grannule could reduce SP?IL-6 and TNF-?'s contents in serum of rats,reduce COX-2 and TRPV1 m RNA expression in both dorsal root ganglion and lung tissue,and reduce TRPV1 channel protein expression in both dorsal root ganglion and lung tissue.It may be one of the mechanisms of Chanqin Granule in treating postinfectious cough.
