ObjectiveThe objectives of the study are to use the clinical randomized controlled trials method by observing clinical efficacy of premenstrual acne with fire-type liver stagnation through the combination of internal consumption and external treatment, and to analyse the improvement of acne with internal consumption and external treatment, and to adjust the advantages of menstruation, and to compare between the consumption of chinese medicine or Traditional Chinese Medicine(TCM)mask,in order to explore an effective comprehensive treatment method for the disease.MethodThe medical records were collected from March 2017 to December 2018,based on the cases from selected patients with premenstrual acne problems, who had visited the outpatient clinic of YK Acupuncture&TCM Centre in Malaysia. The 90 female patients with premenstrual acne problem were randomly selected, and divided into three treatment categories, which include Chinese Medicine for internal use+Chinese Medicine Mask Group,Chinese Medicine Mask Group, and Control Group.Chinese Medicine for internal use+Chinese Medicine Mask Group was using self-made Qingganxiaocuo pill plus self-made Qingrejiedu TCM mask;Chinese Medicine Mask Group was using self-made Qingrejiedu TCM mask;and Control Group was using Longdan Xiegan Tang pill. The patients in each groups had went through three treatment cycles and at the end of treatment, the comparison before and after the treatments were observed, which include skin lesion morphology, skin lesion volume, skin lesion color, menstrual status and other indicators;clinical efficacy, and patient acceptance.Results1.The acne was mainly found at the age from 21 to 30 years old, accounting for 48.88%of 90 patients;followed by age from 31 to 40 years old, accounting for 37.78%;age from 14 to 20 years old patients are the least, accounting for 13.33%.2.In the Chinese Medicine for internal use+Chinese Medicine Mask Group, and Control Group, there was no significant difference in the age distribution, duration of disease, and symptoms of acne before treatment(P>0.05).3.After the first menstrual cycle of treatment, there was no statistically significant difference in the degree of improvement in symptoms in all three groups(P>0.05).4.After the second menstrual cycle of treatment, the Chinese Medicine for internal use+Chinese Medicine Mask Group were obviously higher than the Chinese medicine Mask Group(?~2=6.7873,P=0.009<0.01).5.After the third menstrual cycle of treatment,in the comparison of the degree of improvement in symptoms in all three groups, the Chinese Medicine for internal use+Chinese Medicine Mask Group is significantly higher than Chinese Medicine Mask Group(?~2=6.7948,P=0.033<0.01).6.There is a significant difference in the scores of lesions before and after treatment in all three groups, which include Chinese Medicine for internal use+Chinese Medicine Mask Group(Z=-4.476,P<0.01),Chinese Medicine Mask Group(Z=-3.728,P<0.01),and Control Group(Z=-4.665,P<0.01).7.There is a significant difference in the number of skin lesions before and after treatment in all three groups, which include Chinese Medicine for internal use+Chinese Medicine Mask Group(Z=-4.728,P<0.01),Chinese Medicine Mask Group(Z=-4.051,P<0.01),and Control Group(Z=-4.496,P<0.01).8.There is a significant difference in the color scores of skin lesions before and after treatment in all three groups, which include Chinese Medicine for internal use+Chinese Medicine Mask Group(Z=-4.741,P<0.01),Chinese Medicine Mask Group(Z=-4.436,P<0.01),and Control Group(Z=-4.823,P<0.01).9.There is a significant differences in the scores of menstrual conditions before and after treatment in all three groups, which include Chinese Medicine for internal use+Chinese Medicine Mask Group(Z=-4.811,P<0.01),Chinese Medicine Mask Group(Z=-4.579,P<0.01),and Control Group(Z=-4.514,P<0.01).10.There is a significant difference in the total scores of symptoms before and after treatment in all three groups, which include Chinese Medicine for internal use+Chinese Medicine Mask Group(Z=-4.708,P<0.01),Chinese Medicine Mask Group(Z=-3.682,P<0.01),and Control Group(Z=-4.629,P<0.01).11.There is a significant difference in the efficacy score in all three groups.By comparing with the two groups, the Chinese Medicine for internal use+Chinese Medicine Mask Group has higher efficacy score than the Chinese Medicine Mask Group(P=0.002<0.05).The remaining two pairs were not statistically significant.12.There were significant differences in the changes in the condition after three cycles of treatment and three months after stopping treatment in all three groups, which include Chinese Medicine for internal use+Chinese Medicine Mask Group(Z=-4.028,P<0.01),Chinese Medicine Mask Group(Z=-3.366,P<0.01),and Control Group(Z=-3.617,P<0.01).13.In the comparison of the treatment courses required for the onset of action in all three groups, the Chinese Medicine for internal use+Chinese Medicine Mask Group was shorter than the Control Group(P=0.032<0.05),the remaining two pairs were not statistically significant.14.In the comparison of the treatment courses required for the cure in all three groups, the Chinese Medicine for internal use+Chinese Medicine Mask Group was shorter than the Control Group(P=0.014<0.05),the remaining two pairs were not statistically significant.15.In the comparison of the treatment methods in all three groups, the Chinese Medicine for internal use+Chinese Medicine Mask Group was significantly higher than the Chinese Medicine Mask Group(?~2=4.0431,P=0.044<0.05),the remaining two pairs were not statistically significant.ConclusionI.The Chinese Medicine for internal use+Chinese Medicine Mask Group,Mask Group, and Control Group have exact curative effects in improving the morphology of skin lesions, the number of lesions, and the color of skin lesions.2.Oral Chinese medicine combined with mask to treat fire type premature acne with liver stagnation is effective to improve the severity of acne, and to adjust the effect on menstruation.3.The best results were obtained after three courses of treatment in all three groups. Most patients often had recurrence after stopping treatment for three months.4.In the course of treatment and cure, the Chinese Medicine for internal use + Chinese Medicine Mask Group often takes the shortest time and the patient has the highest degree of recognition.
