Objective: To compare the in vitro fertilization outcomes of patients with or without endometriosis and establish a new mouse model of endometriosis via simulating syngenic menstrual endomentrium.Furthermore,to observe the effect of Bu Shen Huo Xue Recipe on the reproductive outcomes of EM rats models,and discuss the possible mechanisms.Methods: A retrospective study was conducted at Shanghai Ninth People's Hospital Affiliated to Shanghai Jiaotong University School of Medicine during the period from 2014 to 2016.Infertile women due to either endomentriosis or tubal factors were enrolled.The main outcomes were: the endocrine profiles during ovarian stimulation,duration of stimulation,the totol of gonadotropin dosage,the number of oocytes retrieved,egg maturation,fertilaztion rate,the number of high-quailty embryos,the implantation and clinical pregnancy rate.Mature(approximately 8-week-old)female C57BL/6 mice was used to establish a new mouse model of EM.We developed a syngeneic mouse model of endometriosis using mouse menstrual donor tissue to inoculate the peritoneum of recipient mice.Four weeks after establishing mouse models of endometriois,all rats were sacrificed to observe the ectopic tissues implanted in the pelvic.After successful duplicating models,all rats were randomly divided into four groups: the model group,blank control group,Chinese medicine group and western medicine group.The rats in Chinese medical group were given a gavage of Bu Shen Huo Xue recipe while model control group and blank control group were given normal saline.The rats in the western medicine group were given a gavage of gestrione.After 3 weeks treatment,all female rats were caged with male rats at 2:1 ratio.After 18 days of pregnancy,all female rats were checked for pregnant.The methods of medical intervation and the way in dividing groups were as previously reported to explore the poosible mechanism by which Bu Shen Huo Xue Recipe treated the EM-related infetilty.All rats were sacrificed at proestrus stage to obtain the ovarian issues.Bilateral ovaries were removal,one of which was stained by HE while another one was immediately frozen by liquid nitrogen.3 ovarin tissues from each group were randomly detected for the gene expression by microarray gene protein technique.The differiential gene expressions among 3 groups were further analyzed by means of Go annotations,KEGG pathway and Venn analysis,and explained the biological significance and potential mechanisms.Result: A total of 375 patients were recruited in our study.There was no difference in terms of maternal age,duration of infertility,body mass index,basic serum hormone profiles and antral follicle count.Compared with the control group,patients with endometriosis had lower number of oocytes retrieved,lower number of top-quality embryos,however,the oocytes retrieval rate,the maturation and fertilization rate were comparable between groups.Furthermore,we successfully developed a new mouse model of EM,from which we observed that Bu Shen Huo Xue Recipe had positive effect on the reproduction.HE staining results showed that the rate of atresia follicle and the apoptosis index were both significantly higher in the model rats groups than the other treatment groups.However,the rates of these events could patially adversed by the treatment of traditional Chinese medicine.The results form the GEPC identified that Bu Shen Huo Xue Recipe regulated the inflammatory gene expressions and upregulated the anti-apoptotic gene expression such as Bcl-2.Conclusions: BuShen Ho Xue Recipe has benefit effects on the EM-associated infertility.The possible mechanism behind this is that traditional Chinese medicine maybe ameliorate the inflammation damage to the ooytes and granulosa cells via improving the inflammory ovary micro-environment.
