Studying On The Formulation And Clinical Application Of Danggui Liuhuang Decoction Through Data Analysis Method For Medical Cases

ObjectiveIn this paper,the medical case of Danggui Liuhuang Decoction was used as the research data.By means of data analysis,the characteristics of Danggui Liuhuang Decoction about compatibility,syndrome factors,indications and clinical changes were studied,which provides the basis for its clinical application.MethodsThe literature of the paper comes from the database of Chinese Periodicals,such as CNKI,Wanfang database,VIP database.The result finds that 446 cases of 475 cases in 735 articles meet the standard.The statistical methods involved in this paper include frequency statistics,gold segmentation,association analysis,factor analysis,cluster analysis and so on.The establishment of this paper is divided into two parts: the first is to use the traditional Chinese medicine inheritance platform software to organize data to provide data for frequency statistics and correlation analysis;the second is to use Excel2003 to establish a database of high-frequency drugs and high-frequency symptoms to provide data for spss cluster analysis and factor analysis.Results 1.Analysis of general information(1)There is no significant difference between men and women in sex.The youngest is 1.the oldest is 90.the average age is 39.1.(2)The main diseases of TCM are sweat syndrome,night sweats,diabetes,internal injury,fever,menopausal symptoms,insomnia,gall disease,palpitation,aphtha,and drench syndrome and so on.(3)The main syndromes of TCM involve Yin deficiency and fire,Qi and yin deficiency,dampness heat accumulation,yin deficiency and dampness heat,blood stasis obstruction,kidney yin deficiency,liver kidney yin deficiency and wind heat syndrome and so on.(4)The main diseases of modern medicine are type 2 diabetes,menopausal syndrome,hyperthyroidism,oral ulcers,tuberculosis,chronic pharyngitis,autonomic nerve dysfunction and so on.(5)The four nature of Danggui Liuhuang Decoction is mainly "cold and warm",and the five tastes is "sweet and bitter".2.Analysis of pulses and syndromesAccording to the golden section method,more than 61.8% are the core symptoms,and the percentage between 38.2% and 61.8% is the main symptom.The percentage greater than or equal to 61.8% of the symptoms are thin pulse,red tongue.The percentages in 38.2%-61.8% were night sweats,pulse numbers,upset,thin tongue,and yellow fur.The percentage of spontaneous sweating and fatigue is 37.4%,which is close to 38.2%,so it can also be included.3.Analysis of formulation(1)The results of frequency statistics: The result of single herb showed that the frequency of Rehmannia glutinosa was the highest.The result of 2 herb combinations showed that the frequency of "Rehmannia Rehmannia" and "Angelica" was the highest.The result of the 3 herb combinations showed that the frequency of "Rehmannia,angelica,Scutellaria" was the highest.(2)The results of correlation analysis: The result of single herb showed that "Rehmannia glutinosa" was the most closely related to the whole party.The result of 2 herb combinations showed that "Rehmannia Rehmannia,Angelica" was the most closely related to the whole party.The result of 3 herb combinations showed that "Rehmannia,angelica,Scutellaria" was the most closely related to the whole party.(3)The results of factor analysis about high frequency herbs: Danggui Liuhuang Decoction was reduced to F1(Huanglian,Scutellaria,Cortex Phellodendri,Radix Angelicae Sinensis,Angelica sinensis)and factor F4(Angelica sinensis,Rehmannia glutinosa,Rehmannia glutinosa,Huang Qi).(4)The dose results of herbs about Danggui Liuhuang Decoction: The highest frequency dose nodes of Rehmannia glutinosa was 15 grams,and the highest frequency dose nodes of Radix Angelicae,radix rehmanniae,Scutellaria,Rhizoma Coptidis and cypress were 10 grams,and the maximum frequency of Astragalus was 30 grams.The average dose:Astragalus22.5g>Rehmannia glutinosa 15.4g>Rehmannia glutinosa14.1g>Angelica sinensis 11.9g> Scutellaria 10.1g> Phellodendron 9.6g> Rhizoma Coptidis 7.4g.4.Analysis of syndrome factors(1)The syndrome differentiation results of factor analysis about high frequency symptoms: Heat syndrome consists of factor F2 and F4;yin deficiency syndrome consists of factor F9,F12,F17,F18 and F16;phlegm damp syndrome involves factor F3;damp and hot syndrome consists of factor F13 and F11;Qi deficiency syndrome consists of factor F6,F8 and F11.(2)The syndrome differentiation results of cluster analysis based on factor analysis about high frequency symptoms: C2(yin deficiency and damp heat syndrome);C3(yin deficiency fire syndrome);C4(syndrome of phlegm dampness);C6(Qi deficiency syndrome);C9(yin deficiency syndrome);C11(kidney yang deficiency syndrome);C12(yin deficiency syndrome);C13(Qi deficiency syndrome).5.Analysis of addition and subtraction herbs(1)The results of frequency about addition and subtraction herbs: Oyster,licorice,floating wheat,Schisandra chinensis,keel,Atractylodes macrocephala and Radix Paeoniae Alba are the most common addition and subtraction herbs.(2)The results of frequency about addition and subtraction herbs classification: Astringency herb,qi invigorating herb,tranquilizing herb,nourishing yin herb and diuresis herb are the most common addition and subtraction herb categorys.(3)The results of the cluster analysis about addition and subtraction herbs: C1(hawthorn divine MALT),C2(Radix Fructus Schisandrae),C3(Sanba Mezhenko),C4(Radix Ophiopogon),C5(Radix ophiopogonae rhubarb),C6(peony bark of Radix ophiopogonae),C7(sour jujube nut Bai Ziren Bupleurum Bupleurum),C8(glutinous rice root dried oyster keel floating small wheat ephedra root),and C10(Anemarrhena)Tortoise shell),C11(Coix Paeonia lactiflora and Radix Paeoniae),C12(Salvia miltiorrhiza),C13(Glycyrrhiza cinnamomi),C14(Atractylodes rhizome),C15(tuckahoe Alisma pericarp),and C16(Gardenia gypsum).Conclusions1.The clinical application of Danggui Liuhuang Decoction has broken through the limitation of "sweat syndrome" in the past,and now it involves every system disease,which manifests the principle of "treating different diseases with the same treatment".2.The best indication of Danggui Liuhuang Decoction: Night sweat,upset,sweating,fatigue,red tongue,thin yellowish fur,thin pulse."The red tongue and the number of veins" are the main points of the syndrome differentiation.Insomnia,dry mouth,body heat,pulse string,dry stool,mental fatigue,less tongue fur and greasy tongue coating were the secondary clinical manifestations of Danggui Liuhuang Decoction.The current textbook says that it treats "fever and night sweating",and "fever" should be "body heat",which is more in keeping with clinical practice.3."Radix Rehmanniae and Angelica sinensis" is the key to the compatibility of yin deficiency about Danggui Liuhuang Decoction.The prescription compatibility of Danggui Liuhuang Decoction is to seek Yin in blood,and to treat heat and purge fire as well as replenishing qi and consolidating the exterior.This prescription reuses tonic herbs,especially Astragalus.4.The syndrome factors of Danggui Liuhuang Decoction mainly involves Yin deficiency,Qi deficiency and dampness heat.The prescription is characterized by Yin deficiency syndrome and damp heat syndrome existing at the same time.5.Danggui Liuhuang Decoction should be emphasized in clinical practice with the change of pathogenesis: if qi is not satisfied,add "Yam,Hawthorn,Schisandra" in Qiweiduqi Pill;if yin deficiency and sweat are worse,add "Mulberry leaf,Ligustrum lucidum";if accompanied by yin deficiency and dryness,add “Liquid Radix,Scrophulariaceae,and Rhubarb” in Zeng Cheng Tang Cheng Qi Tang;if it is accompanied by negative vulvar fever,add “Dendrobium,Armor,Cortex Mouttuynia” in Artemisia annua Liaojia Tang;if accompanied by Yin Huo Huo Wang,increase "Yinmu,Tortoiseshell" in Buyin Pills;if the yin deficiency is disturbing,add the “Suanzaoren,Boziren,Bupleurum” in Tianwang Buxin Pills;if accompanied by deficiency,Jiayue San or Yu Pingfeng scattered;if it is the deficiency of yin and yang,add the "Glycyrrhiza,Guizhi,Baimao,Jujube" in the soup of Huangqi Jianzhong;if accompanied by yin-deficiency heat-fired poisons,add “Huozi,gypsum” in Huanglian Jiedu Decoction,and so on.
